基于"虚、痰、瘀"辨治糖尿病周围神经病变

糖尿病周围神经病变(DPN)多因消渴日久,肺失转输、脾失运化、肾失固摄导致多脏虚损,精气生化乏源,气血运行不足,四肢末节失濡,久病痰浊四溢,流于四肢,阻滞关节脉络;血运凝滞,阻滞四肢末梢,经脉壅滞不通,临床常见由虚、痰、瘀导致的四肢麻木、疼痛、发凉、蚁行感等症状.本文通过对糖尿病周围神经病变危险因素及病理生理的总结,在中医基础理论指导下,探讨中医"虚、痰、瘀"等病理因素在糖尿病周围神经病变的发病中发挥的作用,提出"补益气血、化痰祛湿、活血化瘀"的治法,常用黄芪、当归、半夏、茯苓、桃仁、红花等药物辨证施治,从根本上缓解临床症状,为糖尿病周围神经病变的中医预防及治疗提供临床思路.
Differential Treatment of Diabetic Peripheral Neuropathy based on"Deficiency,phlegm and Blood stasis"
Diabetic peripheral neuropathy(DPN)is often caused by extinction of thirst for a long time,lung transfusion,spleen transfu-sion,kidney loss,deficiency of vital energy,deficiency of qi and blood,loss of extremities,excessive phlegm and turbid phlegm,flow in the extremities and block the veins of the joints.The blood circulation is stagnant,the extremities are blocked,and the meridians are blocked.Clinical symptoms such as limb numbness,pain,coldness and ant sensation caused by deficiency,phlegm and blood stasis are common.Based on the summary of the risk factors and pathophysiology of diabetic peripheral neuropathy,and under the guid-ance of the basic theory of traditional Chinese medicine,this paper discusses the role of TCM pathological factors such as deficiency,phlegm and blood stasis in the pathogenesis of diabetic peripheral neuropathy.This paper puts forward the treatment method of"tonify-ing qi and blood,resolving phlegm and removing dampness,activating blood circulation and removing blood stasis".Radix Astragali,Angelica sinensis,Pinellia ternata,Poria cocos,peach kernel,safflower and other drugs are often used for syndrome differentiation and treatment.Fundamentally alleviate clinical symptoms and provide clinical ideas for TCM prevention and treatment of diabetic peripheral neuropathy.
