The Effect of Thoracoscopic Reduction and Internal Fixation on Prognosis of Elderly Rib Fractures
Objective To evaluate the impact of thoracoscopic reduction and internal fixation on the rehabilitation process,fracture healing,and functional recovery of elderly patients with rib fractures.Methods 112 elderly patients with rib fractures admitted to People's Hospital of Ningxia Hui Autonomous Region from June 2020 to June 2023 were selected and divided into a conventional group(traditional reduction and internal fixation surgery)and a thoracoscopic group(thoracoscopic assisted reduction and internal fixation surgery)based on different surgical methods,with 56 cases in each group.The rehabilitation progress,fracture healing,and functional recovery between the two groups were compared.Results The surgical time,incision length,intraoperative bleeding,mobilization time,extubation time,length of hospital stay,and fracture healing time of the thoracoscopic group were significantly shorter than those of the conventional group(P all<0.05);and the visual analogue scale(VAS)scores at 1 day,3 days,and 7 days after surgery of the thoracoscopic group were also significantly lower than those of the conventional group(P all<0.05);Compared with preoperative conditions,forced vital capacity(FVC),peak expiratory flow(PEF),maximal voluntary ventilation(MVV)and oxygen saturation(SaO2)significantly increased in both groups 14 days after surgery(P all<0.05),while the increase in the thoracoscopic group was significantly greater than that in the conventional group(P<0.05);The total incidenceof complications in the thoracoscopic group was significantly lower than that in the conventional group(7.14%vs 30.36%,P<0.05).Conclusion Compared with traditional reduction and internal fixation surgery for the treatment of elderly rib fractures,thoracoscopic assisted reduction and internal fixation surgery has the strengths of shorter surgical time,smaller incision,faster postoperative recovery,and fewer complications.Patients have better fracture healing and respiratory function improvement effects.
