Analysis of pelvic and abdominal lymph node metastasis and prognosis in patients with advanced ovarian cancer after satisfactory tumor cell reduction surgery
Objective To analyze the lymph node metastasis and prognosis of patients with advanced ovarian cancer after satisfac-tory cytoreductive surgery.Methods A total of 120 patients with advanced ovarian cancer who underwent satisfactory cytoreductive surgery were retrospectively analyzed.By comparing the postoperative lymph node status,number of metastatic lymph nodes(MLN),lymph node metastasis rate(LNR)and logarithmic ratio of positive lymph nodes(LODDS)of patients after surgery,Kaplan-Meier univariate analysis and multivariate Cox regression survival analysis were used to predict the independent prognostic factors after surgery.According to the re-ceiver operating characteristic(ROC)curve,the predictive ability of different lymph node indexes for the postoperative survival of patients with advanced ovarian cancer was compared.Results Among the 120 patients with advanced stage,71 cases(59.20%)had negative lymph nodes and 49 cases(40.80%)had positive lymph nodes.Multivariate analysis showed that positive LN status,MLN,LNR and LODDS were independent risk factors for OS.Compared with lymph node status and the number of metastatic lymph nodes,LODDS had the highest AUC in predicting the 1,3 and 5 years OS(P<0.05).Conclusion This study demonstrates that positive LN status,MLN,LNR,and LODDS are the predictors of OS in patients with ovarian cancer.Among them,LODDS is the best prognostic factor for predicting the mortality of patients with ovarian cancer after satisfactory cytoreductive surgery.
