Clinical significance of the expression of macrophage M1/M2 polarization-related factors in gingival tissue for patients with severe chronic periodontitis
Objective To investigate the expression and clinical significance of macrophage M1/M2 polarization-related fac-tors in the gingival tissue of patients with severe chronic periodontitis.Methods The damaged gingival tissues of 96 patients with severe chronic periodontitis were collected as the research group,and 80 healthy gingival tissues(gingival tissue covered by the crown of impact-ed teeth)were collected as the control group during the same time period.).The expression levels of macrophage M1/M2 polarization-related factors in the gingival tissue of the two groups were compared.After 6 months of treatment,the treatment effects of patients with severe chronic periodontitis were evaluated and divided into the poor curative effect group and the good curative effect groups.The clinical data of patients were compared.The related factors affecting the treatment effect were analyzed by Logistic multivariate regression.A re-ceiver operating characteristic curve(ROC)was used to analyze the predictive value of the macrophage M1/M2 polarization-related factors in the gingival tissue on the treatment effect of patients with chronic periodontitis.Results The expression levels of inducible ni-tric oxide synthase(iNOS)mRNA,signal transduction and activator of transcription 1(STAT1)mRNA,arginase Ⅰ(Arg Ⅰ)mRNA and signal transduction and activator of transcription 6(STAT6)mRNA in the gingival tissues of the research group were higher than those in the control group(P<0.05).The incidence of poor efficacy in 96 cases of severe chronic periodontitis patients was 12.50%af-ter 6 months of treatment.The expressions of attachment loss(AL),simplified tartar index(CI-S),iNOS mRNA,STAT1 mRNA,Arg ⅠmRNA and STAT6 mRNA in the poor curative effect group were higher than those in the good curative effect group(P<0.05).Logistic multivariate analysis showed that CI-S,iNOS mRNA,STAT1 mRNA and Arg Ⅰ mRNA were the risk factors affecting the treatment effi-cacy of patients with severe chronic periodontitis(P<0.05).ROC analysis showed that the optimal cut-off points for predicting the therapeutic effect of patients with chronic periodontitis based on the expression levels of iNOS mRNA,STAT1 mRNA,and Arg I mRNA in gingival tissues were 1.16,1.21 and 0.87.The area under the curve(AUC)was 0.772,0.776 and 0.812.Conclusion The expres-sion levels of macrophage M1/M2 polarization-related factors iNOS mRNA,STAT1 mRNA and Arg Ⅰ mRNA in gingival tissue have high predictive value for the treatment effect of patients with severe chronic periodontitis and have certain reference value in clinical practice.
