目的 探索缺氧程度及血清CRP水平在新生儿缺氧缺血性脑病(HIE)神经发育中的预测价值.方法 选取未行亚低温治疗的HIE患儿作为病例组(n=49),同期健康新生儿作为对照组(n=35),收集受试新生儿出生6 h内血清检测CRP水平、1 min Apgar评分及30 d Gesell评分.分析CRP水平、Apgar评分与Gesell评分的相关性,ROC曲线评估CRP水平及Apgar评分在HIE新生儿神经发育中的预测价值.结果 对比健康新生儿,HIE患儿血清CRP表达水平升高,与Gesell评分呈显著负相关性(P<0.05);对比健康新生儿,HIE患儿1 min Apgar评分明显降低,与Gesell评分呈显著正相关性(P<0.05);新生儿出生6 h内CRP水平联合1 min Apgar评分对HIE患儿神经发育具有较高预测价值(AUC=0.91).结论 出生6 h内血清CRP水平及1 min Apgar评分可用于预测新生儿脑缺氧缺血后神经发育情况.
The predictive value of hypoxia degree and serum CRP level in the neurodevelopment of HIE
Objective To explore the predictive value of the degree of hypoxia and the level of serum CRP in the neurological development of neonatal hypoxic ischemic encephalopathy(HIE).Methods The neonates with HIE who did not undergo the hypother-mia treatment were selected as the case group,while the healthy neonates from the same period served as the control group.The levels of serum CRP within 6 hours after birth,1-minute Apgar scores,and 30-day Gesell scores were collected.The correlation between CRP levels,Apgar scores,and Gesell scores was analyzed.ROC curve were used to assess the predictive value of CRP levels and Apgar scores in the neurological development of HIE.Results Compared with healthy neonates,the expression level of serum CRP in HIE neonates was increased and showed a significant negative correlation with Gesell scores(P<0.05).The 1-minute Apgar scores of HIE neonates were significantly lower than those of healthy neonates and showed a significant positive correlation with Gesell scores(P<0.05).The levels of CRP within 6 hours after birth combined with the 1-minute Apgar scores had high predictive value for neurological development in HIE(AUC=0.91).Conclusion Serum CRP levels within 6 hours after birth and the 1-minute Apgar scores can be used to predict the neurological development of neonates after suffering from hypoxic-ischemic brain injury.
