3D-Slicer软件在脑膜瘤术前规划中的应用

目的 探讨3D-Slicer软件在脑膜瘤术前规划中的应用价值.方法 回顾性分析应用3D-Slicer软件辅助手术治疗的26例脑膜瘤患者临床资料.术前完善头颅CT、增强核磁共振(MRI)、磁共振血管成像(MRA)、头颈部CT血管造像(CTA)等检查,应用3D-Slicer软件对肿瘤的位置、形态、毗邻的血管神经及静脉窦等进行多模态融合处理,重建出三维虚拟化可视图像,模拟手术入路,制定精准的术前规划进行手术治疗并通过改良Rankin量表(mRs)进行评分.结果 26例脑膜瘤手术患者均无死亡,术后病理检查明确为脑膜瘤.其中,肿瘤全切除23例,占88.46%,Simpson Ⅰ级切除15例,Simpson Ⅱ级切除8例;次全切除3例,占11.54%,均进行Simpson Ⅲ级切除.术后1月随访,增强MRI均未见肿瘤复发,其中mRs评分0分有23人,1分有1人,2分有2人.结论 在脑膜瘤手术治疗方面,3D-Slicer软件可以帮助制定详细的术前规划,减少手术并发症,提高肿瘤的切除率,改善患者预后.
Application of 3D-Slicer software in preoperative planning of meningiomas
Objective To explore the application value of 3D-Slicer software in the preoperative planning of meningioma.Methods A retrospective analysis was conducted on the clinical date of 26 patients with meningioma who underwent surgical treatment with the help of 3D-Slicer software.Preoperative head CT,contrast-enhanced MRI,MRA,head and neck CTA and other examinations were performed.The 3D-Slicer software was used to perform multimodal fusion on the location,morphology,adjacent blood vessels,nerves and venous sinuses of the tumor,reconstruct 3D virtual visual images,simulate surgical approaches,and formulate accurate preoperative planning,and the modified Rankin Scale(mRs)was used to score.Results 26 patients who underwent the meningioma surgery did not die and the pathological reports clearly made the diagnosis of meningioma.Among them,23 cases(88.46%)underwent complete tumor resec-tion,15 cases underwent Simpson grade Ⅰ resection,and 8 cases underwent Simpson grade Ⅱ resection.There were 3 cases of subtotal resec-tion,accounting for 11.54%,and all of them underwent Simpson grade Ⅲ resection.At the one month follow-up after surgery,no tumor recurrence was found on enhanced MRI.Through the mRs score,23 people were scored on a score of 0(normal life),1 person on a score of 1,and 2 points for two people.Conclusion In the surgical treatment of meningioma,3D-Slicer software can help to formulate detailed preoperative plans,reduce surgical complications,improve the resection rate of tumors and enhance prognosis.
