摘要
目的:探讨医共体模式下延伸护理在晚期前列腺癌癌痛患者中的应用效果.方法:选取 2020 年 8 月 1 日～2021 年 1 月1 日收治的 100 例晚期前列腺癌癌痛患者为研究对象,按照随机数字表法将患者分为对照组和观察组各 50 例,对照组实施传统出院护理,观察组实施医共体模式下延伸护理;比较两组死亡、出院前 1 d及出院 3 个月后生存质量[采用癌症患者生命质量测定量表(QLQ-C30)]、疼痛程度[采用数字等级评定量表(NRS)、有效控制疼痛率]、服药依从性、焦虑程度[采用美国前列腺癌患者焦虑状况测评工具(MAX-PC)汉化版].结果:出院 3 个月后,两组QLQ-C30 评分高于出院前 1 d(P<0.05),且观察组高于对照组(P<0.05);出院 3 个月后,观察组NRS疼痛等级、疼痛控制有效率、服药依从性评分均优于对照组(P<0.01);出院 3 个月后,观察组MAX-PC各维度评分低于对照组(P<0.01).结论:医共体模式下延伸护理可以提高晚期前列腺癌患者服药依从性,有效控制疼痛,从而改善其生存质量,缓解焦虑情绪.
Abstract
Objective:To investigate the effect of extended nursing under the medical community model on patients with advanced pros-tate cancer pain.Methods:A total of 100 patients with advanced prostate cancer pain admitted from August 1,2020 to January 1,2021 were selected,they were divided into control group and observation group according to the random number table method with 50 in each group.The control group received traditional discharge nursing,while the observation group received extended nursing under the medical commu-nity model.The difference in scores of quality of life questionnaire-C30(QLQ-C30),the numerical rating scale(NRS),effectively control-ling pain rate,medication compliance and Chinese version of Memorial Anxiety Scale for Prostate Cancer(MAX-PC)were compared be-tween the two groups at 1d before discharge and at 3 months after discharge.Results:At 3 months after discharge,the QLQ-C30 scores in both groups were higher than those at 1 d before discharge(P<0.05),and the observation group was higher than the control group(P<0.05).At 3 months after discharge,there were significant differences in the NRS pain grades effectively controlling pain rate,medication compliance between the two groups(P<0.01).At 3 months after discharge,the scores of various dimensions of MAX-PC in observation group were significantly lower compared with those in control group(P<0.05).Conclusion:Extended nursing under community model can improve the medication compliance and effectively control the pain,thus improving the quality of life and relieving the anxiety of patients with advanced prostate cancer pain.
