摘要
目的:探讨eCASH理念下的进阶式肺康复护理对慢性阻塞性肺疾病(COPD)急性加重期患者的影响.方法:选取 2022年5 月1 日~2023 年9 月30 日收治的87 例COPD急性加重期患者作为研究对象,随机分为对照组43 例和干预组44 例,对照组实施常规肺康复护理,干预组实施eCASH理念下的进阶式肺康复护理;比较两组干预前后疾病活动度[包括膈肌活动度(DE)、膈肌增厚分数(DTF)、气道阻力(Raw)、气道峰压(PIP)、气道平台压(Pplat)],临床指标及并发症发生情况.结果:干预 14 d,两组DE、DTF均高于干预前(P<0.05),Raw、PIP、Pplat均低于干预前(P<0.05),且干预组优于对照组(P<0.01);干预组镇静达标时间、机械通气时间、ICU入住时间短于对照组(P<0.01),肌力评分高于对照组(P<0.01);干预组并发症发生率低于对照组(P<0.05).结论:eCASH理念下的进阶式肺康复护理能有效控制COPD急性加重期患者疾病活动度,改善呼吸力学相关指标,缩短镇静达标、机械通气及ICU入住时间,提高肌力水平,防止不良事件发生.
Abstract
Objective:To explore the application effect of advanced pulmonary rehabilitation nursing under the concept of eCASH on pa-tients with acute exacerbation of COPD.Method:A total of 87 patients with acute exacerbation of COPD received from May 1,2022 to Sep-tember 30,2023 were selected as the subjects of this study and randomly divided into control group and intervention group.In the control group,43 patients received routine pulmonary rehabilitation nursing measures,while in the intervention group,44 patients received ad-vanced pulmonary rehabilitation nursing measures under the CASH concept.Disease activity including diaphragm activity(DE),diaphragm thickening fraction(DTF),airway resistance(Raw),peak airway pressure(PIP),and airway plateau pressure(Pplat),clinical indicators and complications were compared between the two groups before and after intervention.Results:After 14 days of intervention,the DE and DTF in the intervention group were higher than those in the control group(P<0.01),while Raw,PIP,and Pplat were lower than those in the control group(P<0.01).The intervention group had shorter sedation time,mechanical ventilation time,and ICU stay time than the con-trol group(P<0.01),and higher muscle strength scores than the control group(P<0.01).The incidence of complications such as ventila-tor-associated pneumonia,delirium,accidental detachment of the duct,and ICU acquired weakness in the intervention group was lower than that in the control group(P<0.05).Conclusion:Advanced pulmonary rehabilitation nursing under the eCASH concept can effectively con-trol disease activity of patients with acute exacerbation of COPD,improve respiratory mechanics related indexes,shorten sedation stand-ards,mechanical ventilation and ICU stay time,improve muscle strength level,and prevent adverse events.
