中医特色护理配合穴位贴敷在中风后肩手综合征患者中的应用

摘要
目的:探讨中医特色护理配合穴位贴敷在中风后肩手综合征(SHS)患者中的应用效果.方法:选择 2022 年 3 月 1 日～2023 年 3 月1 日收治的150 例中风后SHS患者作为研究对象,将患者随机分为A组、B组和C组各50 例,均实施常规护理,其中A组给予穴位贴敷,B组给予中医特色护理,C组给予穴位敷贴联合中医特色护理;比较三组干预前后疼痛程度[采用疼痛视觉模拟评分法(VAS)]、上肢功能[采用简化版Fugl-Meyer 评分量表(FMA)]、负性情绪[采用沃里克-爱丁堡积极心理健康量表(WEMWBS)、焦虑自评量表(SAS)]、日常生活能力[采用日常生活活动能力(ADL)量表]和生活质量[采用健康调查简表(SF-36)].结果:干预 2、4 周,三组VAS评分均低于干预前(P<0.05),且C组低于A、B组(P<0.05,P<0.01);干预后,三组FMA、ADL量表、SF-36 评分均高于干预前(P<0.05),且C组高于A、B组(P<0.05,P<0.01);干预后,三组WEMWBS、SAS评分均低于干预前(P<0.05),且C组低于A、B组(P<0.05,P<0.01).结论:将中医特色护理配合穴位贴敷应用于中风后SHS患者护理中,可促进患者上肢运动功能及日常生活能力的恢复,同时对疼痛程度、负性情绪的减轻具有积极作用,从而有效提高患者生活质量.
Abstract
Objective:To study the effect of traditional Chinese medicine(TCM)characteristic nursing combined with acupoint applica-tion in patients with post-stroke shoulder-hand syndrome(SHS).Methods:A total of 150 post-stroke SHS patients admitted from March 1,2022 to March 1,2023 were selected as the study subjects.According to the random method,the patients were randomly divided into Group A,B,and C,with 50 patients in each group.Each group received routine care,with Group A receiving acupoint application,Group B receiv-ing TCM characteristic nursing,and Group C receiving acupoint application combined with TCM characteristic nursing.The pain levels u-sing the visual analogue scale(VAS),upper limb function using the simplified Fugl-Meyer Assessment(FMA),negative emotions using the warwick-edinburgh mental well-being scale(WEMWBS),Self-rating anxiety scale(SAS),daily living ability using the activities of daily living(ADL),and quality of life using the Health Survey Short Form(SF-36)were compared before and after intervention in three groups.Results:After 2 and 4 weeks of intervention,the VAS scores of all three groups were lower than those before intervention(P<0.05),and scores of group C was lower than those of groups A and B(P<0.05,P<0.01).After intervention,the FMA,ADL,and SF-36 scores of the three groups were higher than those before intervention(P<0.05),and scores of Group C was higher than those of Groups A and B(P<0.05,P<0.01).After intervention,the WEMWBS and SAS scores of the three groups were lower than those before intervention(P<0.05),and scores of Group C was lower than those of Groups A and B(P<0.05,P<0.01).Conclusion:The application of TCM character-istic nursing combined with acupoint application in the nursing of patients with post-stroke SHS can promote the recovery of upper limb mo-tor function and activities of daily living,relieve pain and negative emotions,and effectively improve the patients' quality of life.
