摘要

目的:探讨电针结合低频电刺激对脑卒中偏瘫患者下肢功能的影响,分析下肢功能重建效果的影响因素,并研究相关护理对策.方法:选取 2022 年 1 月1 日~2023 年1 月31 日收治的162 例脑卒中偏瘫患者为研究对象,均接受电针结合低频电刺激干预,连续干预 4 周.采用Fugl-Meyer运动功能量表(FMA)评估患者下肢功能重建效果,并收集患者临床资料,比较不同临床资料脑卒中偏瘫患者FMA评分,分析影响患者下肢功能重建的相关因素,制订护理对策.结果:干预 4 周后,162 例患者FMA评分高于干预前(P<0.05);年龄≥60 岁、入院时美国国立卫生研究院卒中量表(NIHSS)评分≥21 分、合并糖尿病、未结合规律性运动康复训练的脑卒中偏瘫患者干预4 周后FMA评分分别低于年龄<60 岁、入院时NIHSS评分<21 分、未合并糖尿病、结合规律性运动康复训练的患者(P<0.05);经多元线性回归分析显示,年龄≥60 岁、入院时NIHSS评分≥21 分、合并糖尿病是脑卒中偏瘫患者下肢功能重建效果的危险因素(P<0.05),结合规律性运动康复训练是保护因素(P<0.05);经相关性分析显示,脑卒中偏瘫患者下肢功能重建效果与年龄、入院时NIHSS评分、糖尿病合并情况呈负相关(P<0.05),与结合规律性运动康复训练呈正相关(P<0.05).结论:电针结合低频电刺激对改善脑卒中偏瘫患者下肢功能具有积极意义,其下肢功能重建效果可能与患者年龄、入院时NIHSS评分、糖尿病合并情况、是否结合规律性运动康复训练有关.

Abstract

Objective:To explore the effect of electroacupuncture combined with low-frequency electrical stimulation on lower limb func-tion in stroke hemiplegic patients,analyze the influencing factors of lower limb function reconstruction effect,and study relevant nursing strategies.Methods:All 162 stroke hemiplegic patients admitted from January 1,2022 to January 31,2023 were selected as the study sub-jects,all of whom received electroacupuncture combined with low-frequency electrical stimulation intervention for 4 consecutive weeks.The Fugl-Meyer Motor Function Assessment(FMA)was used to evaluate the effectiveness of lower limb function reconstruction in patients,and clinical data was collected to compare the FMA scores of stroke hemiplegic patients with different clinical data.Relevant factors affecting lower limb function reconstruction in patients were analyzed,and nursing strategies were formulated.Results:After 4 weeks of intervention,the FMA scores of 162 patients were higher than before intervention(P<0.05).After 4 weeks of intervention,the FMA scores of stroke hemiplegia patients aged≥60 years,with NIHSS score≥21 at admission,with diabetes,and without regular exercise rehabilitation training were lower than those of patients aged<60 years,with NIHSS score<21 at admission,without diabetes,and with regular exercise rehabili-tation training(P<0.05).Multivariate linear regression analysis showed that age≥60 years,NIHSS score≥21 at admission,and diabetes were risk factors for lower limb function reconstruction in stroke patients with hemiplegia(P<0.05),and regular exercise rehabilitation training was protective factor(P<0.05).Conclusion:Electroacupuncture combined with low-frequency electrical stimulation has positive significance in improving lower limb function in stroke patients with hemiplegia,and the effect of lower limb function reconstruction may be related to the patient's age,NIHSS score at admission,diabetes complications,and whether regular exercise rehabilitation training is com-bined.
