Effect of the Mode of Chronic Disease Management Plus Internet on the Level of Disease Self-management in Outpatients with Hypertension
Objective:To explore the effect of the mode of chronic disease management plus internet on the self-management level in outpatients with hypertension.Methods:150 outpatients with hypertension treated from January 1 to December 31,2022 were selected as the study objects,and they were divided into the control group(72 cases)and the observation group(78 cases)by random number table meth-od.The traditional outpatient management mode was used in the control group,and the mode of chronic disease management plus internet was adopted in the observation group.The self-management level,blood pressure level,living habits,BMI and disease cognition of the pa-tients were compared at different time between the two groups.Results:After six months,the disease self-management level of the patients in observation group was better than that in control group(P<0.01),the systolic blood pressure and diastolic blood pressure were lower than those of the patients in control group(P<0.01),the exercise and disease cognition were higher than those of the control group(P<0.05,P<0.01),the unreasonable diet and BMI>25 were lower than those of the control group(P<0.05,P<0.01).Conclusion:The mode of chronic disease management plus internet can improve the level of disease self-management of outpatients with hypertension and better help them monitor and manage their own diseases so as to improve the quality of life.
