自拟通腑解毒汤对重症胰腺炎胃肠功能障碍疗效观察

摘要
目的:观察自拟通腑解毒汤对重症急性胰腺炎(SAP)胃肠功能障碍(腑实热结型)患者临床疗效.方法:选择2020 年 1 月至2023 年4 月在黔南州中医医院重症医学科的42 例重症急性胰腺炎(SAP)胃肠功能障碍(腑实热结型)患者,随机分为两组,对照组 21 例予西医常规治疗,治疗组 21 例予自拟通腑解毒汤联合西医常规治疗.结果:经过72h的观察发现,治疗组在改善生命体征方面优于对照组(P＜0.05).治疗 7d后,APACHE Ⅱ、Ranson评分、中医证候评分、胃肠道功能评分均较干预前明显降低,并且治疗组显著低于对照组(P＜0.05).同时,治疗组的病死率下降,住院天数缩短,在临床表现上也有所不同,如腹部不适(腹痛腹胀)消失时间更快(P＜0.05).结论:这些结果表明自拟通腑解毒汤能够有效地促进患者生命体征平稳、改善其临床表现并促进胃肠道功能的恢复,从而有效提高患者的生活质量并缩短住院时间.
Abstract
Objective:To observe the clinical efficacy of self-made Tongfu Jiedu Decoction on patients with severe acute pancreatitis(SAP)gastrointestinal dysfunction(Fu-real heat junction type)(TCMZheng:phlegm-heat retention).Methods:We selected 42 patients with severe acute pancreatitis(SAP)gastrointes-tinal dysfunction(TCMZheng:phlegm-heat retention)from January 2020 to April 2023 in the intensive med-icine department of Qiannan state traditional Chinese medicine hospital of our hospital,and randomly divided them into two groups.21 patients in the control group were treated with western medicine regular,and 21 pa-tients in the treatment group were treated with self-made tongfu jiedu decoction combined with western medi-cine regular.Results:After a 72-hour observation,it was found that the treatment group outperformed the control group in terms of improving vital signs(P＜0.05).After 7 days of treatment,the APACHE II score,Ranson score,Traditional Chinese Medicine symptom score,and gastrointestinal function score were significant-ly lower in the treatment group compared to the control group(P＜0.05).Concurrently,the mortality rate in the treatment group decreased,and the length of hospital stay was shortened.There were also differences in clinical manifestations,such as a faster disappearance of abdominal discomfort(abdominal pain and distension)(P＜0.05).Conclusion:The results show that the Tongfu Jiedu Decoction can effectively promote the patient's vital signs,improve the clinical manifestations and promote the recovery of gastrointestinal function,so as to effectively improve the quality of life of patients and shorten the length of hospital stay.
