Impact of Timing of Paracentesis on Inflammatory Status and Immune Function in Severe Acute Pancreatitis Patients with Ascites
Objective:To investigate the impact of the timing of paracentesis on the inflammatory state and immune function in severe acute pancreatitis(SAP)patients with ascites.Methods:Seventy-eight SAP patients with ascites admitted to our hospital from August 2019 to December 2022 were selected and all under-went paracentesis with catheter placement.The control group(39 cases)underwent the procedure 3-7 days after disease onset,while the study group(39 cases)underwent it within 0-2 days.Clinical outcomes,levels of inflammatory factors(IL-8,TNF-α,IL-6,CRP),immune function parameters(immunoglobulins IgG,IgA,IgM,CD3+,CD4+,CD4+/CD8+ratio),and incidence of complications were compared between the two groups.Results:The overall clinical effectiveness rate in the study group(89.74%)was significantly high-er than that in the control group(69.23%,P<0.05).After intervention,the levels of TNF-α,PCT,IL-6,and IL-8 were lower in the study group than in the control group(P<0.05),while IgG,IgA,IgM,CD3+,CD4+,and the CD4+/CD8+ratio were higher in the study group(P<0.05).There was no statisti-cally significant difference in the incidence of complications between the study group(10.26%)and the control group(7.69%,P>0.05).Conclusion:Performing paracentesis with catheter drainage within 0-2 days of disease onset in SAP patients with ascites significantly alleviates the inflammatory state,improves immune func-tion,and demonstrates a definite therapeutic effect without increasing the risk of complications.
