Etiological characteristics,clinical characteristics and influencing factors of prognosis in children with viral meningitis
Objective To explore the etiological characteristics,clinical characteristics and influencing factors of prognosis in children with viral meningitis.Methods A total of 122 children with viral meningitis in the Hebei Children's Hospital were enrolled between January 2019 and June 2022.The etiological characteristics and clinical characteristics were analyzed.According to prognosis,they were divided into poor prognosis group(n=37)and good prognosis group(n=85).The clinical data of patients were collected.The influencing factors of prognosis in children with viral meningitis were analyzed by multivariate logistic regression model.Results Among the 122 children with viral meningitis,there were 93 cases with single virus-specific IgM[mainly on human enterovirus(HEV)-IgM]and 29 cases with multiple virus-specific IgM[mainly on HEV+herpes simplex virus(HSV)-IgM].The main onset age,gender and seasons were 5-9 years,male,and summer and autumn,respectively.The clinical manifestations were characterized by fever.There were some children with headache,malaise,coma and lethargy,and electroencephalogram was normal in most of children.There were significant differences in fever duration,malaise,coma,lethargy and electroencephalogram between poor prognosis group and good prognosis group(x2=12.181,9.919,47.496,13.735,6.724;all P<0.05).The results of binary logistic regression analysis showed that fever ≥15 d(OR=2.162,P=0.007),malaise(OR=2.085,P=0.005),coma(OR=2.622,P=0.001),lethargy(OR=2.241,P=0.002)and abnormal electroencephalogram(OR=1.978,P=0.003)were independent risk factors of prognosis in children with viral meningitis.Conclusions HEV was the main pathogen in children with viral meningitis.The main onset age and seasons were 5-9 years,and summer and autumn,respectively.The main influencing factors of prognosis included fever duration,malaise,coma,lethargy and electroencephalogram.
