Dynamic changes and significance of NLR and blood biochemical indexes in children with severe adenovirus pneumonia
Objective To summarize the dynamic changes of neutrophil/lymphocyte ratio(NLR)and blood biochemical indexes in children with severe adenovirus pneumonia,and analyze its prognostic significance.Methods Seventy children with severe adenovirus pneumonia admitted to the Hebei Children's Hospital from January 2017 to March 2021 were collected.According to the prognosis,they were divided into the cured group(n=57)and the sick-dead group(n=13).NLR values and blood biochemical indexes[alanine aminotransferase(ALT),aspartate aminotransferase(AST)and lactate dehydrogenase(LDH)]of the first and second weeks of the disease course were obtained for retrospective analysis.Results Among the 70 cases,13 cases(18.6%)died and 57 cases(81.4%)were cured.Comparison between groups:there were no significant differences in the levels of NLR,ALT,AST and LDH in the first week of the disease course between the death group and the cure group(all P>0.05),while the levels of NLR,ALT,AST and LDH in the second week of the disease course in the death group were significantly higher than those in the cure group(Z=-5.067,-2.109,-3.000,-4.660;all P<0.05).Intra-group comparison:the levels of NLR in the cured group were significantly decreased in the second week compared with the first week(Z=-6.004,P<0.05),while the levels of AST and LDH had no significant changes compared with the first week(all P>0.05).The levels of NLR were not significantly changed in the second week of the death group compared with the first week(P>0.05),but the levels of ALT and LDH were significantly increased in the second week of the death group compared with the first week(Z=-2.695,-2.795;all P<0.05).There was a significant positive correlation between the level of NLR and the level of LDH and C-reactive protein in children with severe adenovirus pneumonia(r=0.508,P<0.001;r=0.366,P=0.002).Logistic regression analysis of NLR in the second week of the disease course combined with blood biochemical indicators showed that NLR was an independent risk factor for death(P<0.05).Conclusions Dynamic changes in NLR and blood biochemical indicators were related to the prognosis of severe adenovirus pneumonia in children.In particular,NLR was simple and easy to obtain and deserved clinical attention.
