Relationship,between IL-6,CRP and PCT levels and cardiac function and prognosis in elderly patients with chronic heart failure and pulmonary infection
Objective To measure the level of interleukin-6(IL-6),C-reactive protein(CRP)and procalcitonin(PCT)and analyze their relationship with cardiac function and prognosis in elderly patients with chronic heart failure(CHF)complicated with pulmonary infection.Methods The clinical data of 68 elderly patients with CHF complicated with pulmonary infection admitted to the Nantong Third People's Hospital from January 2021 to June 2022 were collected(infected group),and 68 elderly patients with CHF without pulmonary infection were included in control group with gender and age at 1:1 ratio.The differences in serum IL-6,CRP and PCT levels were compared between the two groups.Spearman rank correlation analysis was used to evaluate the correlation between cardiac function New York Heart Association(NYHA)grading and serum indicators in infected patients.Patients in infected group were followed up for 6 months and divided into good prognosis group(n=39)and poor prognosis group(n=29)according to the occurrence of adverse cardiovascular events during follow-up.Receiver operating characteristic(ROC)curve was applied to analyze the predictive value of serum IL-6,CRP and PCT on poor prognosis in infected group.Results The levels of serum IL-6,CRP and PCT in infected group were significantly higher than those in control group,the differences were statistically significant(t=8.245,27.598,15.154;all P<0.001).Spearman rank correlation analysis showed that cardiac function grading was positively correlated with serum IL-6,CRP and PCT levels in infected group(r=0.683,0.597,0.655;all P<0.01).Serum levels of IL-6,CRP and PCT were significantly higher in poor prognosis group than those in good prognosis group,the differences were statistically significant(t=6.748,3.745,7.032;all P<0.001).ROC curve analysis revealed that serum IL-6,CRP and PCT had high predictive value on poor prognosis in infected group(AUC=0.885,0.749,0.889;all P<0.001),and the combination of the three indicators had higher prediction value(AUC=0.957,P<0.001).Conclusions Serum levels of IL-6,CRP and PCT were significantly increased among elderly patients with CHF and pulmonary infection,and the levels of the three indicators were closely related to the cardiac function grading of patients.The detection of the three serum indicators also had a certain role on predicting the poor prognosis of patients.
