病原体靶向高通量测序技术对肺结核的诊断价值

目的 分析病原体靶向高通量测序(tNGS)对肺结核的诊断价值,为临床诊疗提供依据.方法 选取2022年1月-2023年6月海南医学院第二附属医院收治的121例疑似肺结核患者.根据临床诊断标准,分为结核组(n=72)、非结核组(n=46)及混合感染组(n=3),通过留取呼吸道标本并进行分析比较tNGS与传统检测方法(涂片法、培养法)诊断效能差异,且参照对比表型药敏试验(pDST)分析tNGS耐药基因检测效能.结果 结核组tNGS检测与传统检测方法的敏感度、特异度、阳性预测值、阴性预测值及一致率比较,差异均有统计学意义(x2=62.169、26.623、30.117、40.474、64.769,P均<0.001).tNGS耐药基因与pDST对比后显示tNGS对异烟肼、利福平的耐药检测效能一致性较理想(Kappa均>0.40).结论 tNGS技术作为单一检测项目使用时,诊断效能高于传统涂片法、培养法,对疑似肺结核患者的临床诊断和治疗具有较高的临床应用价值.
The diagnosis value of pathogen-targeted next-generation sequencing technology for tuberculosis
Objective Analyzing the diagnostic value of pathogen-targeted next-generation sequencing(tNGS)for tuberculosis.Methods One hundred and twenty-one patients with suspected tuberculosis admitted to the Second Affiliated Hospital of Hainan Medical College from January 2022 to June 2023 were selected.According to the clinical diagnostic criteria,they were divided into the tuberculosis group(n=72),the non-tuberculosis group(n=46)and the mixed infection group(n=3),and the diagnostic efficacy of tNGS was analyzed by taking respiratory specimens and comparing the difference between the diagnostic efficacy of tNGS and that of traditional detection methods(smear and culture),and the efficacy of the detection of tNGS resistance gene was analyzed with reference to the comparative phenotypic drug sensitivity test(pDST).Results The sensitivity,specificity,positive predictive value,negative predictive value and concordance of the tNGS assay in the tuberculosis group were compared with those in the traditional assay;all the differences were statistically significant(c2=62.169,26.623,30.117,40.474,64.769,all P<0.001).The comparison between tNGS resistance genes and pDST showed that the consistency of the resistance detection efficacy of tNGS against isoniazid and rifampicin was more favorable(Kappa both>0.40).Conclusion When tNGS was used as a single test,the diagnostic efficacy was higher than that of traditional smear and culture methods,and it had high clinical value for the clinical diagnosis and treatment of patients with suspected tuberculosis.
