The diagnosis value of pathogen-targeted next-generation sequencing technology for tuberculosis
Objective Analyzing the diagnostic value of pathogen-targeted next-generation sequencing(tNGS)for tuberculosis.Methods One hundred and twenty-one patients with suspected tuberculosis admitted to the Second Affiliated Hospital of Hainan Medical College from January 2022 to June 2023 were selected.According to the clinical diagnostic criteria,they were divided into the tuberculosis group(n=72),the non-tuberculosis group(n=46)and the mixed infection group(n=3),and the diagnostic efficacy of tNGS was analyzed by taking respiratory specimens and comparing the difference between the diagnostic efficacy of tNGS and that of traditional detection methods(smear and culture),and the efficacy of the detection of tNGS resistance gene was analyzed with reference to the comparative phenotypic drug sensitivity test(pDST).Results The sensitivity,specificity,positive predictive value,negative predictive value and concordance of the tNGS assay in the tuberculosis group were compared with those in the traditional assay;all the differences were statistically significant(c2=62.169,26.623,30.117,40.474,64.769,all P<0.001).The comparison between tNGS resistance genes and pDST showed that the consistency of the resistance detection efficacy of tNGS against isoniazid and rifampicin was more favorable(Kappa both>0.40).Conclusion When tNGS was used as a single test,the diagnostic efficacy was higher than that of traditional smear and culture methods,and it had high clinical value for the clinical diagnosis and treatment of patients with suspected tuberculosis.
