KOA合并骨质疏松患者外周血JAK2/STAT3信号通路表达水平及临床意义

目的 分析膝关节骨性关节炎(KOA)合并骨质疏松(OP)患者外周血中,janus酪氨酸激酶2(JAK2)/信号传导和转录激活因子3(STAT3)信号通路表达水平及临床意义.方法 选取2021年1月-2022年12月在南阳市中心医院骨科进行诊治的KOA合并OP患者作为合并组(n=106),另选同期诊查的单纯KOA患者作为对照组(n=106).收集两组的年龄、性别、体质量指数(BMI)等一般资料,检测JAK2、磷酸化JAK2(p-JAK2)、STAT3、磷酸化STAT3(p-STAT3)蛋白表达水平,采用单因素分析和多因素logistic回归分析外周血JAK2/STAT3信号通路表达与KOA合并OP的关系,并采用受试者工作特征(ROC)曲线分析外周血JAK2/STAT3信号通路表达对KOA合并OP的预测价值.结果 单因素分析显示,性别、年龄、BMI指数、住房情况、JAK2、p-JAK2、STAT3、p-STAT3蛋白表达均是KOA合并OP的影响因素,差异均有统计学意义(x2/t=10.930、16.652、9.468、6.861、13.621、12.089、12.850、11.065,P均<0.05).多因素logistic回归分析显示,性别(OR=1.397,95%CI:1.091~1.788)、年龄(OR=1.631,95%CI:1.162~2.289)、BMI指数(OR=2.210,95%CI:1.167~4.187)、住房情况(OR=1.642,95%CI:1.082~2.493)、JAK2 蛋白(OR=2.125,95%CI:1.257~3.594)、p-JAK2 蛋白(OR=2.111,95%CI:1.285~3.466)、STAT3 蛋白(OR=2.088,95%CI:1.320~3.302)和p-STAT3蛋白(OR=2.140,95%CI:1.244~3.684)均是KOA合并OP的独立危险因素(P均<0.05).ROC曲线分析显示,JAK2、p-JAK2、STAT3、p-STAT3蛋白及四者联合检测的曲线下面积(AUC)分别为0.898、0.880、0.897、0.857、0.985,四者联合检测优于单一检测,差异有统计学意义(P<0.05).结论 膝骨性关节炎合并骨质疏松患者外周血JAK2、p-JAK2、STAT3、p-STAT3蛋白均呈高表达,KOA合并骨质疏松可能跟外周血JAK2/STAT3信号通路活跃有关,联合检测对KOA合并骨质疏松具有良好的预测价值.
Expression and clinical significance of JAK2/STAT3 signaling pathway in peripheral blood of KOA patients with osteoporosis
Objective To analyze the expression of janus kinase 2/signal transducers and activators of transcription 3(JAK2/STAT3)signaling pathway in peripheral blood of patients with knee osteoarthritis(KOA)complicated with osteoporosis(OP)and its clinical significance.Methods KOA patients with OP who were treated in the Department of Orthopedics,Nanyang Central Hospital from January 2021 to December 2022 were selected as the study subjects and named as the combined group(n=106),and KOA patients diagnosed during the same period were selected as the control group(n=106).Age,sex,body mass index(BMI)and other general data of the two groups were collected,and JAK2,phosphorylation JAK2(p-JAK2),STAT3,phosphorylation STAT3(p-STAT3)protein expression levels were detected.Univariate analysis and multiple logistic regression were used to analyze the relationship between JAK2/STAT3 signaling pathway expression and KOA combined with OP in peripheral blood.The predictive value of JAK2/STAT3 signaling pathway expression in peripheral blood for KOA combined with OP was analyzed by receiver operating characteristic(ROC)curve.Results Univariate analysis showed that gender,age,BMI,housing condition,JAK2,p-JAK2,STAT3 and p-STAT3 protein expression were the influencing factors of KOA and OP,and the differences were statistically significant(c2/t=10.930,16.652,9.468,6.861,13.621,12.089,12.850,11.065;all P<0.05).Multivariate logistic regression analysis showed that gender(OR=1.397,95%CI:1.091-1.788),age(OR=1.631,95%CI:1.162-2.289),BMI(OR=2.210,95%CI:1.167-4.187),housing situation(OR=1.642,95%CI:1.082-2.493),JAK2 protein(OR=2.125,95%CI:1.257-3.594),p-JAK2 protein(OR=2.111,95%CI:1.285-3.466),STAT3 protein(OR=2.088,95%CI:1.320-3.302)and p-STAT3 protein(OR=2.140,95%CI:1.244-3.684)were independent risk factors for KOA combined with OP(all P<0.05).ROC curve analysis showed that the area under the curve(AUC)of JAK2,p-JAK2,STAT3,p-STAT3 and the combined detection of the four proteins were 0.898,0.880,0.897,0.857 and 0.985,respectively,and the combined detection of the four proteins was better than the single detection,the difference was statistically significant(P<0.05).Conclusions JAK2,p-JAK2,STAT3 and p-STAT3 proteins in peripheral blood of patients with knee osteoarthritis complicated with osteoporosis were highly expressed.KOA complicated with osteoporosis might be related to the active JAK2/STAT3 signaling pathway in peripheral blood.Combined detection had good predictive value for KOA complicated with osteoporosis.
