Characteristics and influencing factors of rifampicin-resistance in patients with pulmonary tuberculosis in a hospital in Bachu county,Xinjiang from 2019 to 2023
Objective To analyze the characteristics of rifampicin-resistance and related risk factors in patients with tuberculosis in a hospital in Bachu county,and provide a scientific basis for the prevention and control of local tuberculosis.Methods A total of 1 198 patients with positive etiological and molecular biological diagnosis in the People's Hospital of Bachu county from July 2019 to August 2023 were collected.The rifampin-resistant and rifampin-sensitive tuberculosis patients were matched by propensity score matching method.After the propensity score matching,92 rifampin-resistant patients and 368 non-rifampin-resistant patients were matched.The clinical baseline data of the patients were analyzed by retrospective cohort study,and the influencing factors of rifampin-resistance in patients with pulmonary tuberculosis were analyzed by logistic regression model.Results Multivariate conditional logistic regression analysis showed that the probability of rifampicin-resistance in retreated patients was 2.09(95%CI:1.28-3.42)times of that in newly diagnosed patients.The probability of rifampicin resistance in smoking patients was 3.86(95%CI:1.29-11.53)times of that in non-smoking patients.The probability of rifampicin-resistance in patients with secondary hyperuricemia was 2.01(95%CI:1.15-3.54)times of that in patients without secondary hyperuricemia.The probability of rifampicin-resistance in patients with respiratory diseases was 2.32(95%CI:1.39-3.87)times higher than that in patients without respiratory diseases.The probability of rifampicin-resistance in patients with lung imaging showing proliferation was 2.64(95%CI:1.34-5.20)times higher than that in patients without proliferation.The above factors were statistically significant between the resistant and non-resistant groups(all P<0.05).Conclusion Among the patients with pulmonary tuberculosis,retreatment,smoking,secondary hyperuricemia,respiratory diseases and pulmonary proliferation were the risk factors for rifampicin-resistance,which should be focused on control and prevention.
