Determination of serum 25(OH)D and analysis of related factors in patients with nephrotic syndrome
Objective To explore the clinical significance of 25-hydroxyvitamin D[25(OH)D]in development and treatment of nephrotic syndrome(NS)by analyzing the correlation between serum 25(OH)D level and clinically relevant indicators and renal pathology in NS.Methods A total of 155 NS patients first diagnosed in the Department of Nephrology,Huazhong University of Science and Technology Union Shenzhen Hospital from September 2013 to August 2019 were collected as NS group,and 80 healthy people examined in our hospital during the same period were in the control group.Clinical data and pathological indicators of patients were collected.Compared the mean 25(OH)D between the two groups.The correlations between 25(OH)D and clinical detection indexes were analyzed.According to the median value of 25(OH)D level,it was divided into high and low level groups.The difference between the two groups and the difference of 25(OH)D level of patients with different pathological types and clinical stages were analyzed.Results The average of the 25(OH)D level in NS group was greatly lower than that in control group,the difference was statistically significant(t=-4.077,P<0.05).Through correlation analysis,25(OH)D level was positively correlated with serum total protein,albumin,and estimated glomerular filtration rate(r=0.321,0.306,0.295;all P<0.05),and negatively correlated with 24 hour urine protein,serum creatinine,and triglyceride(r=-0.256,-0.199,-0.165;all P<0.05).The total protein,albumin,and estimated glomerular filtration rate of the 25(OH)D low level group were significantly lower than those of the high level group,the differences were statistically significant(t=-2.747,-2.665,-2.005;all P<0.05),and the 24 hour urine protein was higher than that of the high level group,the difference was statistically significant(t=-1.953,P<0.05).The comparison of 25(OH)D levels in the four NS pathological types showed no significant differences(P>0.05).In the stage of membranous nephropathy,the expression level of 25(OH)D of stage Ⅱ and Ⅲ significantly decreased compared to stage I,the difference was statistically significant(F=3.343,P<0.05).Conclusion The expression level of 25(OH)D was closely correlated with nephrotic syndrome and could be applied to the patient's condition monitoring,prognosis assessment and clinical treatment.
