Clinical manifestations,chest CT and pathogenic bacteria characteristics in 160 patients with invasive pulmonary fungal infection
Objective To analyze the clinical manifestations,chest CT,pathogen characteristics and prognosis of 160 patients with invasive pulmonary fungal infections(IPFIs).Methods A total of 160 patients with IPFIs admitted to the Meishan Hospital of Traditional Chinese Medicine from January 2020 to April 2023 were selected as observation subjects,and their clinical and chest CT characteristics and pathogenic bacteria characteristics were analyzed.The patients were divided into survival group(n=137)and death group(n=23)according to the survival status at discharge.Baseline data were compared between the two groups,and the correlative factors influencing the prognosis of patients with IPFIs were discussed by logistic regression analysis.Results The main clinical symptoms of 160 patients with IPFIs were fever(88.75%),cough(71.25%)and chest pain(50.63%),and the main physical signs were pulmonary rales(83.13%)and cyanosis(23.75%).The chest CT findings of 160 patients with IPFIs were mainly lamellar/patchy dense shadow(61.25%),hollow shadow(31.25%)and ground-glass opacity change(25.63%).283 strains of pathogenic bacteria were isolated from 160 patients with IPFIs,mainly Candida albicans(47.35%),followed by Candida tropicalis(20.14%)and Aspergillus(18.02%).There were no statistically significant differences in age,gender,underlying diseases,the proportion of liver injury,blood creatinine,albumin and white blood cell count between survival group and death group(all P>0.05),but the proportion of acute kidney injury in survival group was significantly lower than that in death group(x2=6.669,P<0.001),while the glomerular filtration rate,platelet and hemoglobin were significantly higher than those in death group(t=7.437,5.996,6.025;all P<0.001).Logistic regression analysis showed that the incidence rate of acute kidney injury,glomerular filtration rate and hemoglobin level were correlative factors affecting the prognosis of patients with IPFIs(all P<0.05).Conclusion Candida albicans infection was the main infection in patients with IPFIs,and the clinical characteristics and chest CT manifestations were not specific,and the incidence rate of acute kidney injury,glomerular filtration rate and hemoglobin level were correlative factors affecting the poor prognosis of patients.
