外周血TGF-β1、miR-146a及胃粘膜TFF1、GKN2表达与Hp感染慢性胃炎病情程度的关系

目的 探究外周血转化生长因子-β1(TGF-β1)、miR-146a及胃粘膜三叶草因子1(TFF1)、胃动蛋白2(GKN2)表达与幽门螺旋杆菌(Hp)感染慢性胃炎病情程度的关系,为临床诊疗提供参考.方法 选取2019年11月-2022年11月郑州市第七人民医院收治的62例Hp感染慢性胃炎患者作为病例组,另选62例在本院接受治疗且未感染Hp的慢性胃炎患者作为对照组,分析两组患者的外周血TGF-β1、miR-146a及胃粘膜TFF1、GKN2表达情况.根据胃炎评估量表(OLGA)/胃炎肠化生评估量表(OLGIM)分期对病例组进行分组:0~Ⅱ期患者纳入轻度组,Ⅲ~Ⅳ期纳入重度组,探究组间TGF-β1、miR-146a相对表达量、胃粘膜TFF1、GKN2表达及其mRNA水平差异.结果 与对照组相比,病例组的外周血TGF-β1、miR-146a相对表达量均升高,差异均有统计学意义(t=-7.475、-6.883,P均<0.05).病例组的胃粘膜TFF1、GKN2阳性率低于对照组,TFF1 mRNA、GKN2 mRNA水平低于对照组,差异均有统计学意义(x2/t=4.212、4.146、13.760、18.236,P均<0.05).与轻度组患者相比,重度组患者的外周血TGF-β1、miR-146a相对表达量均升高,差异均有统计学意义(t=-10.392、-24.192,P均<0.05).重度组胃粘膜TFF1、GKN2阳性率低于轻度组,TFF1 mRNA、GKN2 mRNA水平低于轻度组,差异均有统计学意义(x2=4.353、5.035、7.398、6.302,P均<0.05).受试者工作特征(ROC)曲线显示,TGF-β1、miR-146a相对表达量、胃粘膜TFF1 mRNA、GKN2 mRNA水平诊断重度组患者的曲线下面积(AUC)分别为0.817、0.761、0.776、0.838,各指标联合诊断的AUC为0.900,高于各指标单独诊断(P<0.05).结论 伴Hp感染的慢性胃炎患者TGF-β1、miR-146a表达水平升高,TFF1、GKN2基因表达阳性率及其mRNA相对表达水平均降低,且与患者病情严重程度有关,具有一定的诊断价值.
Relationship between the expression of TGF-β1,miR-146a in peripheral blood and TFF1,GKN2 in gastric mucosa and the severity of chronic gastritis infected with Hp
Objective To explore the relationship between the expression of transforming growth factor-β1(TGF-β1),miR-146a and gastric mucosa trefoil factor 1(TFF1),gastrokine 2(GKN2)in peripheral blood and the severity of chronic gastritis infected by Helicobacter pylori(Hp).Methods Sixty-two patients with Hp-infected chronic gastritis admitted to the Seventh People's Hospital of Zhengzhou from November 2019 to November 2022 were selected as the case group,and 62 patients with chronic gastritis who received treatment in the hospital and were not infected with Hp were selected as the control group.The expression of TGF-β1,miR-146a and TFF1,GKN2 in peripheral blood and gastric mucosa of the two groups of patients were analyzed.The case groups were grouped according to the operative link on gastritis assessment(OLGA)/operative link on gastritis intestinal metaplasia assessment(OLGIM)stage:patients in stage 0-Ⅱ were included in the mild group,and the severe group in stage Ⅲ-Ⅳ.The differences in TGF-β1,miR-146a expression,TFF1,GKN2 expression and their mRNA levels were explored.Results Compared with control group,the relative expression levels of TGF-β1 and miR-146a in peripheral blood of patients in case group were increased(t=-7.475,-6.883;all P<0.05).The positive rate of gastric mucosa TFF1,GKN2 in case group was lower than those in control group,and the mRNA level of TFF1,GKN2 in case group was lower than those in control group(x2/t=4.212,4.146,13.760,18.236;all P<0.05).Compared with the mild group,the relative expression levels of TGF-β1 and miR-146a in peripheral blood of the severe group were increased(t=-10.392,-24.192;all P<0.05).The positive rate of TFF1,GKN2 in gastric mucosa in severe group was lower than those in mild group,and the mRNA level of TFF1,GKN2 in mild group was lower than those in mild group(x2/t=4.353,5.035,7.398,6.302;all P<0.05).The area under the curve(AUC)values of the receiver operating characteristic(ROC)curves of TGF-β1,miR-146a relative expression levels,and TFF1/GKN2 mRNA levels in gastric mucosa in severe group were 0.817,0.761,0.776 and 0.838,respectively,and the AUC value of the combined diagnosis of these indexes was 0.900,which was higher than that of the separate diagnosis of each index(P<0.05).Conclusion In patients with chronic gastritis complicated with Hp infection,the expression levels of TGF-β1 and miR-146a were increased,while the positive expression rate of TFF1,GKN2 gene and its relative mRNA expression level were decreased,which was related to the severity of the disease and had certain diagnostic value.
