肾细胞癌患者自体NK细胞治疗效果及外周淋巴细胞亚群、miR-122-5p和miR-29a-3p变化

目的 分析肾细胞癌患者自体自然杀伤(NK)细胞治疗后效果及外周血淋巴细胞亚群、微小RNA(miR)-122-5p和miR-29a-3p变化,为临床治疗提供参考.方法 纳入2023年3月-2024年1月甘肃省武威肿瘤医院接受治疗的60例肾细胞癌患者,根据治疗方案分为观察组(n=31)与对照组(n=29).对照组接受常规化疗方案治疗,观察组接受常规化疗联合自体NK细胞治疗.比较两组的治疗效果;比较两组治疗前后的外周血淋巴细胞亚群(CD3+、CD4+、CD8+、CD4+/CD8+)、肿瘤标志物[癌胚抗原(CEA)、糖类抗原 125(CA125)]及 miR-122-5p、miR-29a-3p 表达;比较两组的不良反应.结果 观察组治疗有效率高于对照组,差异有统计学意义(x2=4.348,P<0.05).治疗后观察组 CD3+、CD4+、CD4+/CD8+水平均高于对照组(t=2.448、4.794、4.500),CD8+、CEA、CA125 水平低于对照组(t=4.812、4.129、3.823),差异均有统计学意义(P均<0.05).治疗后观察组miR-122-5p和miR-29a-3p表达均高于对照组,差异均有统计学意义(t=2.860、3.526,P均<0.05);两组不良反应比较差异无统计学意义(x2=0.201,P>0.05).结论 自体NK细胞治疗对肾细胞癌患者具有较好的治疗效果,能够改善免疫功能、降低肿瘤标志物表达,其机制可能与miR-122-5p、miR-29a-3p 相关.
Effect of autologous NK cell therapy and the changes of peripheral lymphocyte subsets,miR-122-5p and miR-29a-3p in patients with renal cell carcinoma
Objective To analyze the effect of autologous natural killer(NK)cell therapy and the changes of peripheral blood lymphocyte subsets,microrna(miR)-122-5p and miR-29a-3p in patients with renal cell carcinoma.Methods 60 patients with renal cell carcinoma admitted to Gansu Wuwei Tumour Hospital from March 2023 to January 2024 were included and divided into observation group(n=31)and control group(n=29)according to the treatment plan.The control group received conventional chemotherapy,and the observation group received conventional chemotherapy combined with autologous NK cell therapy.The therapeutic effect of the two groups was compared.The subsets of peripheral blood lymphocytes(CD3+,C D4+,CD8+,CD4+/CD8+),tumor markers[carcino embryonic antigen(CEA),carbohydrate antigen 125(CA125)]and the expressions of miR-122-5p and miR-29a-3p were compared between the two groups before and after treatment.The adverse reactions of the two groups were compared.Results Compared with the control group,the effective rate was better in the observation group(x2=4.348,P<0.05).Compared with the control group,the levels of CD3+,CD4+,CD4+/CD8+in observation group were all increased after treatment(t=2.448,4.794,4.500;all P<0.05).Compared with the control group,the levels of CD8+,CEA and CA125 in observation group were all decreased after treatment(t=4.812,4.129,3.823;all P<0.05).Compared with the control group,the expressions of miR-122-5p and miR-29a-3p in the observation group were all increased after treatment(t=2.860,3.526;all P<0.05).There was no significant adverse reaction between the two groups(x2=0.201,P>0.05).Conclusion Autologous NK cell therapy had a good therapeutic effect on patients with renal cell carcinoma,which could improve the immune function and inhibit the expression of tumor markers,and the mechanism might be related to miR-122-5p and miR-29a-3p.
