Colorectal cancer screening status and factors influencing colonoscopy compliance in Panyu district,Guangzhou
Objective To explore the status of colorectal cancer screening and the influencing factors of colonoscopy compliance in Panyu district,Guangzhou. Methods The data of the population aged 40-74 years participating in colorectal cancer screening in Panyu district,Guangzhou in 2021 were analyzed. The factors influencing colonoscopy compliance were explored using a logistic regression model, and the receiver operating characteristic(ROC)curve of colonoscopy compliance was drawn. Results The positive rate of colorectal cancer in the initial screening in Panyu district,Guangzhou in 2021 was 19.64%(1 156/5 887),and the positive colonoscopy rate was 28.37%(328/1 156).The colonoscopy rate was highest in the age group of 40-<45 years old(x2=719.422,P<0.05).The detection rate of non-advanced adenomas was highest in the age group of 40-<45 years old, while the detection rates of advanced adenomas and colorectal cancer were highest in the age group of 55-<74 years old,the differences were statistically significant when compared with the other age groups(all P<0.05).Multivariate logistics regression analysis showed that female(OR=1.347,P<0.05),only positive fecal occult blood test or both positive in the initial screening(OR=2.430,2.081;all P<0.05)were favorable factors for colonoscopy compliance.Old age(OR=0.974,P<0.05)and fewer follow-up times(OR=0.873,P<0.05)were unfavorable factors for colonoscopy. Conclusions The compliance of colonoscopy among residents in Panyu district of Guangzhou who were positive in the initial screening for colorectal cancer was not high.Targeted measures should be taken to improve the compliance of colonoscopy, especially for males, elderly patients, those with only high-risk factor questionnaire positive results in the initial screening, and those with few follow-up times.
