多层螺旋CT动态增强及后处理技术在结肠癌患者术前分期和诊断中的意义

摘要
目的:探讨多层螺旋CT(MSCT)动态增强及后处理技术对结肠癌诊断及术前分期的评估价值.方法:选取本院 2020 年 7 月至 2022 年 7 月就诊的 128 例疑似结肠癌患者作为研究对象,入院后均行MSCT动态增强及后处理技术检查,以手术病理结果为"金标准",比较MSCT动态增强及联合后处理技术诊断结果、诊断效能,并分析其对结肠癌术前分期与术后病理分期的一致性.结果:与单独MSCT动态增强比较,联合后处理技术对于结肠癌诊断灵敏度(95.29%,81/85)、准确度(95.31%,122/128)较高,漏诊率(4.71%,4/85)较低,差异有统计学意义(P＜0.05);MSCT动态增强检查及联合后处理技术T分期与术后病理学检查结果一致性优(Kappa值=0.794、0.953),N分期与术后病理学检查结果一致性优(Kappa值=0.793、0.935),M分期与术后病理学检查结果一致性分别为良好和优(Kappa值=0.748、0.919).结论:MSCT动态增强联合处理技术可提高结肠癌诊断准确性,为临床术前分期提供参考,以针对性制定手术方案,改善预后.
Abstract
Objective:To explore the diagnostic value of dynamic contrast-enhanced multi-slice spiral computed tomography(MSCT)and post-processing techniques for the diagnosis and preoperative staging of colon cancer.Methods:A total of 128 suspected colon cancer patients treated in our hospital from July 2020 to July 2022 were selected as the study objects.After admission,all patients underwent MSCT dynamic enhancement and post-processing technology examination.The surgical pathological results were taken as the "gold standard" to compare the diagnostic results and diagnostic efficacy of MSCT dynamic enhancement and combined post-processing techniques,and analyze their consistency with preoperative staging and postoperative pathological staging of colon cancer.Results:Compared with MSCT dynamic enhancement alone,the sensitivity(95.29%,81/85)and accuracy(95.31%,122/128)of combined post-treatment were higher,and the missed diagnosis rate(4.71%,4/85)was lower,and the differences were statistically significant(P＜0.05).MSCT dynamic enhancement examination and combined post-processing techniques Kappa values for concordance between T staging and postoperative pathologic findings were 0.794 and 0.953,respectively,Kappa values for concordance between N staging and postoperative pathologic findings were 0.793 and 0.935,respectively,and Kappa values for concordance between M staging and postoperative pathologic findings were 0.748 and 0.919,respectively.Conclusions:MSCT dynamic enhancement combined with processing technology can improve the diagnostic accuracy of colon cancer,and provide reference for preoperative staging,so as to make targeted surgical plan and improve prognosis.
