动态心电图检查在冠心病无症状心肌缺血诊断中的应用价值

摘要
目的:探究动态心电图检查在临床诊断冠心病无症状心肌缺血中的应用价值.方法:选择 2019 年 1 月至2022 年 6 月本院收治的 98 例心肌缺血冠心病患者为研究对象.按照心肌缺血是否无症状,将其分为观察组(n = 50,无症状心肌缺血患者)和对照组(n =48,有症状心肌缺血患者),分析两组患者心率变异指标、心电图ST段变化、心肌缺血变异性.结果:观察组动态心电图检查结果,均为窦性心搏RR间期、相邻RR间期差值均方根、RR间期平均值标准差各评分,均比对照组低,差异均有统计学意义(P＜0.05);观察组ST段下移范围、下移保持时间、ST段下移次数评分均比对照组显著增高,差异均有统计学意义(P＜0.05);观察组心肌缺血阈变异性、心肌缺血发作、心肌缺血时间评分均比对照组高,差异均有统计学意义(P＜0.05).结论:在冠心病无症状心肌缺血患者中应用动态心电图检查能有效增强心肌缺血的确诊率,且为临床提供了诊治依据,具有更高的临床价值.
Abstract
Objective:To investigate the clinical value of holter electrocardiography in the diagnosis of asymptomatic myocardial ischemia in coronary heart disease.Methods:A total of 98 coronary heart disease patients who admitted to our hospital from January 2019 to June 2022 were selected as research subjects.According to their myocardial ischemia with or without symptom,they were divided into an observation group(n =50,myocardial ischemia without symptom patients)and a control group(n =48,myocardial ischemic with symptom patients).Statistical analysis of heart rate variation index,ST segment change of ECG and myocardial ischemic variability were analyzed and compared between two groups.Results:The scores of ST segment downshift range,retention time of ST segment downshift and ST segment downshift frequency in observation group were higher than those in control group,the differences were statistically significant(P＜0.05);the scores of all sinus cardiogram RR intervals,RMS difference between adjacent RR intervals and mean standard deviation of RR intervals in observation group were lower than those in control group,the differences were statistically significant(P＜0.05);Myocardial ischemia time,myocardial ischemia threshold variability and myocardial ischemia attack time score in observation group were higher than those in control group,the differences were statistically significant(P＜0.05).Conclusion:The application of holter electrocardiograph in myocardial ischemia without symptom patients with coronary heart disease can effectively enhance the detection rate of myocardial ischemia,and provide clinical basis for diagnosis and treatment,and has higher clinical value.
