摘要

目的:探讨骨折手术后输血反应的相关影响因素,并构建输血反应风险预测模型.方法:选取 2022 年 1月至2023 年5 月在本院实施骨折手术治疗后,发生输血反应的40 例患者为研究对象,纳入研究组.选取同期在本院进行骨折手术,但未发生输血反应的 50 例患者作为对照,纳入对照组.比较两组患者相关临床资料,采用多因素非条件logistics回归分析方法对影响输血反应发生因素进行分析.结果:二元多因素Logistics回归分析方法分析结果显示,输血史、变态反应史、发血至输血时间>0.5h、C 反应性蛋白(CRP)水平≥9.065mg/L,均是影响骨折手术患者输血后发生输血反应的独立危险因素(P<0.05).根据该回归分析结果构建输血反应发生风险预测模型,对发生输血反应风险的预测效果良好.结论:骨折手术后输血治疗患者发生输血反应与多方面因素有关,临床可提前有针对性地对患者采取个体化预防措施,降低患者的输血反应发生风险.

Abstract

Objective:To explor the related influencing factors of transfusion reactions after blood transfusion in fracture surgery,and to establish the corresponding risk prediction model.Methods:A total of 40 patients with transfusion reactions after blood transfusion in fracture surgery from January 2022 to May 2023 in our hospital were selected as study group and 50 patients with no transfusion reactions after blood transfusion in fracture surgery in our hospital during the same period were selected in the control group.The basic data between two groups were analyzed and statistical compared,and the influencing factors of transfusion reactions were summarized.Results:Regression analysis of binary multivariate logistic showed that a history of blood transfusion,allergies,duration of bleeding to transfusion>0.5h,and level of CRP≥9.065mg/L were all independent risk factors for transfusion reactions in patients undergoing fracture surgery after blood transfusion(P<0.05).The prediction model constructed based on regression analysis results has a good predictive effect on transfusion reactions after blood transfusion.Conclusion:Transfusion reactions after blood transfusion in fracture surgery are related to multiple factors,and clinical prevention can be taken in advance and targeted to reduce the risk of transfusion reactions in patients after blood transfusion in fracture surgery.
