静脉溶栓联合机械取栓治疗对前循环大血管闭塞性急性缺血性脑卒中患者的临床疗效

摘要
目的:探讨静脉溶栓联合机械取栓治疗,对前循环大血管闭塞性急性缺血性脑卒中(AIS-LVO)患者的临床疗效.方法:收集 2021 年 5 月至 2022 年 5 月本院收治的前循环AIS-LVO患者 95 例作为研究对象.按治疗方法不同将其分为联合组(n =54,采取静脉溶栓+机械取栓治疗)和对照组(n =41,仅采取静脉溶栓治疗).对两组治疗后临床疗效、脑血管开通、血流灌注、美国国立卫生院卒中量表(NIHSS)评分及预后进行统计学分析.结果:联合组患者临床总有效率(92.59%),显著高于对照组(75.61%),差异有统计学意义(P＜0.05);联合组血管再通率(79.63%)及血管灌注≥2b级占比(72.22%),均显著高于对照组 60.98%、51.22%,差异均有统计学意义(P＜0.05);联合组预后不良事件发生率(5.56%)低于对照组(19.51%),mRS评分也明显低于对照组,差异均有统计学意义(P＜0.05).结论:前循环AIS-LVO静脉溶栓联合机械取栓治疗疗效佳,可改善患者神经功能,提高血管再通率,对预后有积极作用.
Abstract
Objective:To analyze clinical effects of intravenous thrombolysis combined with mechanical thrombectomy in treatment of patients with acute ischemic stroke with large vessel occlusion(AIS-LVO)in the anterior circulation.Methods:A total of 95 caces of patients with AIS-LVO admitted to our hospital from May 2021 to May 2022 were collected as research subjects.They were divided into combination group(n =54,intravenous thrombolysis+mechanical thrombectomy)and a control group(n = 41,intravenous thrombolysis)according to different treatment.The clinical effective rate,vascular opening rate,proportion of vascular perfusion mTICI≥2b,incidence of adverse events were compared between two groups after treatment.Results:The total clinical effective rate(92.59%)in the combination group was significantly higher than that in the control group(75.61%),and the difference was statistically significant(P＜0.05);the vascular opening rate(79.63%)and the proportion of vascular perfusion mTICI≥2b(72.22%)in the combination group were significantly higher than those in the control group(60.98%,51.22%),and the difference was statistically significant(P＜0.05);the incidence of adverse events in the combination group(5.56%)was lower than that in the control group(19.51%),and the mRS score was also significantly lower than that of the control group,and the difference was statistically significant(P＜0.05).Conclusion:Intravascular thrombolysis combined with mechanical thrombectomy for AIS-LVO in the anterior circulation has good curative effect,which can further improve the neurological function of patients,increase the rate of vascular recanalization,and has a positive effect on prognosis.
