摘要
目的:探讨后路椎体间融合术(PLIF)与经椎间孔入路椎体间融合术(TLIF)对腰椎退行性病变患者临床疗效、术后疼痛及腰椎功能的影响.方法:选取2021 年4 月至2023 年2 月本院收治的104 例腰椎退行性疾病患者作为研究对象.按照手术方法,将其分为观察组(n=58,行TLIF治疗)和对照组(n=46,行PLIF治疗).比较两组患者手术相关指标、疼痛程度、腰椎功能及并发症发生率.结果:观察组患者手术时间短于对照组,植骨融合率高于对照组,差异均有统计学意义(P＜0.05);两组患者术中出血量、住院时间比较,差异均无统计学意义(P＞0.05);两组患者术后McGill疼痛评估量表(MPQ)评分和Oswestry功能障碍指数问卷表(ODI)评分均下降,观察组术后 6 个月MPQ评分和ODI评分均低于对照组,差异均有统计学意义(P＜0.05);两组并发症发生率比较,差异无统计学意义(P＞0.05).结论:TLIF治疗腰椎退行性病变具有较好的手术疗效,可有效缓解患者术后疼痛感,促进腰椎功能恢复,并降低并发症发生率.
Abstract
Objective:To explore the effects of posterior interbody fusion(PLIF)and transforaminal approach interbody fusion(TLIF)on the clinical efficacy,postoperative pain and lumbar spine function in patients with lumbar degenerative disease.Methods:A total of 104 patients with the presence of lumbar degenerative diseases admitted to our hospital from April 2021 to February 2023 were selected in this study.They were divided into observation group(n=58,received TLIF treatment)and control group(n=46,received PLIF treatment)according to the surgical methods.The surgery-related indexes,pain degree,lumbar function and complication rate of the two groups were compared between the two groups.Results:The operation time of observation group was shorter than that of control group,and the bone graft fusion rate was higher than that of control group,the differences were statistically significant(P＜0.05);The difference in intraoperative bleeding and hospitalization time between the two groups was not statistically significant(P＞0.05);The McGill pain questionnaire(MPQ)scores and Oswestry disability index(ODI)scores decreased in both groups after surgery,and MPQ scores and ODI scores in observation group were lower than those in control group at 6 months after surgery,the differences were statistically significant(P＜0.05);Comparison of complications between the two groups was not statistically significant(P＞0.05).Conclusion:TLIF has a good surgical effect in the treatment of lumbar degenerative diseases,can effectively relieve postoperative pain,promote the recovery of lumbar function,and reduce the incidence of complications.
