摘要
目的:探讨急性心力衰竭(AHF)患者住院期间谵妄发生的影响因素.方法:选取 2021 年 10 月至 2022 年10 月于本院住院治疗的 114 例AHF患者为研究对象.根据患者住院期间的谵妄发生情况将其分为发生组(n=28)和未发生组(n=86).比较两组患者的临床资料;采用多因素Logistics回归分析法对AHF患者住院期间谵妄发生的影响因素进行分析.结果:114 例AHF患者住院期间谵妄发生率为 24.56％(28/114).发生组急性生理学与慢性健康状况评分系统-Ⅱ(APACHE-Ⅱ)评分、伴睡眠障碍占比、行机械通气占比、血肌酐(Scr)水平均高于未发生组,差异具有统计学意义(P<0.05).多因素非条件Logistics回归分析结果显示,APACHE-Ⅱ评分高、伴睡眠障碍、行机械通气、高 Scr 水平,均是影响 AHF 患者住院期间谵妄发生的独立危险因素(P<0.05).结论:APACHE-Ⅱ评分高、伴睡眠障碍、行机械通气、高Scr水平均是AHF患者住院期间谵妄发生的影响因素,临床可制定针对性干预措施以降低谵妄发生率.
Abstract
Objective:To analyze the influencing factors of delirium during hospitalization in patients with acute heart failure(AHF).Methods:A total of 114 AHF patients who were hospitalized in our hospital from October 2021 to October 2022 were selected as the study subjects.According to the occurrence of delirium during hospitalization,the patients were divided into an occurrence group(n=28)and a non-occurrence group(n=86).Clinical data between two groups were collected and compared.Multivariate logistic regression analysis was performed to analyze the influencing factors of delirium in AHF patients during hospitalization.Results:The incidence of delirium during hospitalization in 114 AHF patients was 24.56％(28/114).The acute physiology and chronic health status scoring system Ⅱ(APACHE-Ⅱ)score,proportion of sleep disorders,proportion of mechanical ventilation,and serum creatinine(Scr)levels in the occurrence group were all higher than those in the non-occurrence group,the differences were statistically significant(P<0.05).Multivariate logistic regression analysis showed that high APACHE-Ⅱ score,accompanied by sleep disorders,mechanical ventilation,and high Scr levels were all independent risk factors for delirium in AHF patients during hospitalization(P<0.05).Conclusion:High APACHE-Ⅱ score,accompanied by sleep disorders,mechanical ventilation,and high Scr levels are all influencing factors for delirium in AHF patients during hospitalization.Targeted interventions can be developed in clinical practice to reduce the incidence of delirium.
