摘要
目的:探讨不同全身麻醉方式对颌面部恶性肿瘤患者术后镇痛效果及免疫功能的影响.方法:选取 2019年 2 月至 2022 年 8 月本院收治的 80 例颌面部恶性肿瘤患者为研究对象.按照随机分组原则,将其分为 A组(n=20,采取全凭静脉麻醉),B组(n=20,采取静吸复合麻醉)和C组(n=40,采取吸入麻醉).采用统计学方法,比较 3 组患者的镇痛效果及免疫功能水平.结果:3 组患者的术后视觉模拟评分法(VAS)评分比较,差异有统计学意义(P<0.05);术后 2、6、12、24h,3 组患者的术后VAS评分从高到低依次为A、B、C组,差异有统计学意义(P<0.05).术后 24h,3 组患者的CD3+、CD4+、CD4+/CD8+、NK细胞、B淋巴细胞计数比较,差异有统计学意义(P<0.05);3 组患者的CD3+、CD4+、CD4+/CD8+、B淋巴细胞计数从高到低依次为A、B、C组;NK细胞从高到低依次为C、B、A组,差异均有统计学意义(P<0.05).结论:吸入麻醉方式应用于颌面部恶性肿瘤的手术治疗中,可显著提升患者的镇痛效果,对免疫功能影响最小,有利于患者的预后.
Abstract
Objective:To explore the effects of different general anesthesia on postoperative analgesic effect and immune function of patients with oral cancer.Methods:Eighty patients with maxillofacial tumor who received treatment in our hospital from February 2019 to August 2022 were selected as research objects.According to the principle of randomization,they were divided into Group A(n=20,received intravenous anesthesia),Group B(n=20,received intravenous anesthesia combined with inhalation anesthesia)and Group C(n=20,received inhalation anesthesia).The postoperative analgesic effect and immune function level of patients were compared among the three groups by statistical methods.Results:There were statistically significant differences in the analgesic effects among the three groups(P<0.05).The visual analogue scales(VAS)scores at 2,6,12,and 24 hours postoperatively were ranked from high to low in Group A,B and C,respectively,the differences were statistically significant(P<0.05).24 hours after surgery,there were statistically significant differences in the CD3+,CD4+,CD4+/CD8+,NK cell,and B lymphocyte counts among the three groups of patients(P<0.05).The CD3+,CD4+,CD4+/CD8+,and B lymphocyte counts of the three groups of patients were ranked from high to low in Group A,B and C.The NK cells in descending order were Group C,B and A,the differences were statistically significant(P<0.05).Conclusion:In the treatment of oral cancer,inhalation anesthesia can significantly improve the anesthesia effect of patients,minimize the impact on immune function,and be beneficial for the prognosis of patients.
