右美托咪定联合舒芬太尼对老年白内障手术患者的影响

摘要
目的:探讨右美托咪定联合舒芬太尼用于老年白内障手术患者的影响.方法:选取 2017 年 2 月至 2019年 3 月本院收治的 66 例年龄≥60 岁的进行手术治疗的老年白内障患者为研究对象.采用随机数字表法,将其分为联合组(n=33,采用右美托咪定联合舒芬太尼进行麻醉)和对照组(n=33,仅采用舒芬太尼进行麻醉).采用统计学方法,比较两组患者血压、心率、血氧饱和度(SaO2),镇静效果,满意度及不良反应发生率.结果:术前、术中及手术结束时,两组患者血压、心率及SaO2比较,差异无统计学意义(P＞0.05);联合组组内不同时间点血压、心率及SaO2比较,差异无统计学意义(P＞0.05),而对照组组内术中血压与术前相比较,变化较大,差异有统计学意义(P＜0.05).术前和手术结束时,两组患者Ramsay镇静评分比较,差异无统计学意义(P＞0.05);而术中,联合组患者Ramsay镇静评分高于对照组,差异有统计学意义(P＜0.05).联合组患者及医生对手术过程的满意度高于对照组,差异有统计学意义(P＜0.05).两组不良反应发生率比较,差异无统计学意义(P＞0.05).结论:右美托咪定联合舒芬太尼用于老年白内障患者手术麻醉效果较好,安全性好.
Abstract
Objective:To explore the effect of dexmedetomidine combined with sufentanil on elderly patients with cataract.Methods:A total of 66 elderly cataract patients aged≥60 years who underwent surgical treatment in our hospital from February 2017 to March 2019 were selected as the research subjects.By a random number table method,they were divided into combined group(n=33,received dexmedetomidine combined with sufentanil for anesthesia)and control group(n=33,received only sufentanil for anesthesia).The blood pressure,heart rate,blood oxygen saturation(SaO2),sedative effect,satisfaction,and incidence of adverse reactions were compared between the two groups by statistical methods.Results:Before,during,and at the end of operation,there were no statistically significant differences in blood pressure,heart rate,and SaO2 between the two groups(P＞0.05).There were no statistically significant differences in blood pressure,heart rate,and SaO2 at different time points within the combined group(P＞0.05),while in the control group,there was a significant change in intraoperative blood pressure compared to preoperative blood pressure,with a statistically significant difference(P＜0.05).There was no statistically significant difference in Ramsay sedation scales between the two groups of patients before and after surgery(P＞0.05).During surgery,the Ramsay sedation scale of patients in the combined group was higher than that of the control group,and the difference was statistically significant(P＜0.05).The satisfaction of patients and doctors in the combined group with the surgical process was higher than that of the control group,and the difference was statistically significant(P＜0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P＞0.05).Conclusion:Dexmedetomidine combined with sufentanil has good anesthetic effect and safety in elderly cataract patients.
