摘要
目的:探讨责任制护理模式在维持性血液透析(MHD)患者心理状态及负面情绪改善中的应用效果.方法:选取 2020 年 9 月至 2023 年 12 月本院收治的 89 例MHD患者为研究对象.按照干预方案,将其分为观察组(n=46,实施责任制护理模式)和对照组(n=43,实施常规护理干预).比较两组患者负面情绪[焦虑自评量表(SAS)、自我感觉负担量表(SPBS)]、液体平衡情况以及自我管理能力.结果:两组患者干预后SAS、SPBS评分均下降,且观察组低于对照组,差异均有统计学意义(P＜0.05);观察组患者干预后心力衰竭率发生占比低于对照组,而血压达标、体重合格、脱水量合格占比高于对照组,差异均有统计学意义(P＜0.05);两组患者干预后自我管理量表评分均升高,且观察组高于对照组,差异均有统计学意义(P＜0.05).结论:采用责任制护理模式可有效促进MHD患者液体平衡,改善其心理状态,提高其自我管理能力.
Abstract
Objective:To explore the effect of the application of the accountable care model in the improvement of psychological state and negative emotions in maintenance hematodialysis(MHD)patients.Methods:A total of 89 cases of MHD patients admitted to our hospital from September 2020 to December 2023 were selected.The patients were divided into observation group(n=46,application of the accountable care model)and control group(n=43,received conventional nursing intervention),according to intervention schemes.Negative mood[self-rating anxiety scale(SAS),self perceived burden scale(SPBS)],fluid balance,and self-management skills were compared between the two groups.Results:After the intervention,SAS and SPBS scores decreased in both groups,and were lower in observation group than those in control group,the differences were statistically significant(P＜0.05);The percentage of heart failure rate occurrence after the intervention in observation group was lower than that in control group,while the percentage of blood pressure attainment,weight qualification,and dehydration qualification were higher than those in control group,the differences were statistically significant(P＜0.05).The self-management scale scores of both groups increased after intervention,and in observation group was higher than those in control group,the differences were statistically significant(P＜0.05).Conclusion:The use of the accountable care model can effectively control the fluid balance of MHD patients,improve their psychological state,and enhance their self-management ability.
