摘要

目的:探讨计算机断层扫描(CT)与磁共振成像(MRI)诊断膝关节损伤的影像特点及其价值.方法:选取2022 年1 月至2023 年12 月于本院就诊的90 例疑似膝关节损伤患者为研究对象,均行CT、MRI检查.分析两种检查方法的影像特点,以关节镜检查结果作为金标准,分析CT、MRI检查在诊断膝关节损伤中的价值.结果:90 例疑似患者中,经膝关节镜检查发现有 60 例存在膝关节损伤.MRI 诊断膝关节软骨损伤的阳性预测值 98.31%(58/59)、特异度 96.67%(29/30)、灵敏度 96.67%(58/60)、准确度 96.67%(87/90)、阴性预测值 93.55%(29/31)均高于CT检查的 86.21%(50/58)、73.33%(22/30)、83.33%(50/60)、80.00%(72/90)、68.75%(22/32),差异均有统计学意义(P<0.05).结论:膝关节损伤采用CT、MRI检查不同损伤类型均有独特的影像表现,临床可根据影像特点作出准确诊断,且MRI诊断价值优于CT检查.

Abstract

Objective:To explore the imaging characteristics and value of computed tomography(CT)and magnetic resonance imaging(MRI)in the diagnosis of knee joint injury.Methods:Ninety suspected knee joint injury patients treated in our hospital from January 2022 to December 2023 were selected as the research subjects,all of whom underwent CT and MRI examinations.The imaging characteristics of two examination methods were analyzed,using arthroscopic examination results as the gold standard,and the value of CT and MRI examinations in diagnosing knee joint injurieswere analyzed.Results:Among the 90 suspected patients,60 cases were found to have knee joint injuries through knee arthroscopy examination.The positive predictive value,specificity,sensitivity,accuracy,and negative predictive value of MRI in diagnosing knee cartilage injury were 98.31%(58/59),96.67%(29/30),96.67%(58/60),96.67%(87/90),and 93.55%(29/31),respectively,which were higher than those of CT examination 86.21%(50/58),73.33%(22/30),83.33%(50/60),80.00%(72/90),and 68.75%(22/32),respectively,and the differences were statistically significant(P<0.05).Conclusion:Knee joint injuries can be diagnosed accurately based on the unique imaging features of CT and MRI examinations for different types of injuries.MRI has better diagnostic value than CT examinations.
