液基薄层细胞学检查联合阴道镜检查在宫颈癌筛查中的应用

摘要
目的:探讨液基薄层细胞学检查(TCT)联合阴道镜检查在宫颈癌筛查中的应用效果.方法:选取 2021 年1 月至 2023 年 1 月于本院就诊的 2100 例患者为研究对象.所有患者均接受TCT检查.筛选出TCT阳性者,进一步行阴道镜检查及病理活检.以病理活检结果作为"金标准",分析TCT联合阴道镜检查在宫颈癌筛查中的应用效果.结果:2100 例患者经TCT筛查结果显示,80 例患者为TCT阳性,占 3.81%;经病理活检结果显示,80 例TCT阳性者检出宫颈癌 23 例,占 28.75%.以病理检查结果为"金标准",TCT筛查宫颈癌与病理学检查结果的一致性一般(Kappa=0.608,P＜0.001),TCT联合阴道镜检查筛查宫颈癌与病理学检查结果的一致性较强(Kappa=0.907,P＜0.001).结论:对于经TCT检出阳性结果的患者,联合行阴道镜检查能提高对宫颈癌的整体筛查准确率.
Abstract
Objective:To explore the application effect of thin-prep cytology test(TCT)combined with colposcopy in cervical cancer screening.Methods:A total of 2100 patients who visited our hospital from January 2021 to January 2023 were selected as the study subjects.They underwent TCT examination.And the TCT positive patients were selected for further colposcopy and pathological biopsy.Using pathological biopsy as the"gold standard",the application effect of TCT combined with colposcopy in cervical cancer screening was analyzed.Results:After TCT screening,80 out of 2100 patients were TCT positive,accounting for 3.81%.According to pathological biopsy results,23 cases of cervical cancer were detected in 80 TCT positive cases,accounting for 28.75%.Based on pathological results as the"gold standard",the consistency between TCT screening for cervical cancer and the pathological results was general(Kappa=0.608,P＜0.001),while the consistency between TCT combined with colposcopy screening for cervical cancer and the pathological results was strong(Kappa=0.907,P＜0.001).Conclusion:For patients with positive results detected by TCT,combined colposcopy can improve the overall screening accuracy of cervical cancer.
