阴道分泌物常规镜检联合阴道炎五联检在阴道炎诊断中的应用效果

摘要
目的:探讨阴道分泌物常规镜检联合阴道炎五联检在阴道炎诊断中的应用效果.方法:选取 2021 年 1 月至2023 年3 月于本院就诊的234 例疑似阴道炎患者为研究对象.所有患者均接受培养法检测、阴道分泌物常规镜检以及阴道炎五联检.比较两种检查方式对阴道清洁度的检测结果;以培养法作为"金标准",分析阴道分泌物常规镜检、阴道炎五联检以及两者联合检测对阴道炎的检测价值.结果:234 例疑似阴道炎患者经培养法检测确诊为阴道炎 74 例(31.62%),非阴道炎 160 例(68.38%).阴道分泌物常规镜检联合阴道炎五联检对阴道清洁度Ⅲ度、Ⅳ度检出率高于单一检查,差异有统计学意义(P＜0.05);阴道分泌物常规镜检、阴道炎五联检对阴道清洁度检测结果比较,差异无统计学意义(P＞0.05).以培养法为"金标准",阴道分泌物常规镜检联合阴道炎五联检对阴道炎检测的灵敏度、特异度、阴性预测值、准确度、阳性预测值均高于单一检测,差异均有统计学意义(P＜0.05).kappa检验显示,阴道分泌物常规镜检与培养法检测结果的一致性一般(kappa=0.570,P＜0.001),阴道分泌物常规镜检联合阴道炎五联检与培养法检测结果的一致性良好(kappa=0.790、0.950,P＜0.001).结论:阴道分泌物常规镜检联合阴道炎五联检可更准确判断阴道清洁度,提高阴道炎检出率.
Abstract
Objective:To explore the application effect of routine microscopic examination of vaginal secretions combined with quintuple examination of vaginitis in diagnosis of vaginitis.Methods:A total of 234 patients with suspected vaginitis who were admitted to our hospital from January 2021 to March 2023 were selected as research objects.All patients underwent culture method examination,routine microscopic examination of vaginal secretions and quintuple examination of vaginitis.The results of the two examination methods on vaginal cleanliness were analyzed.The culture method was used as the "gold standard" to analyze the detection value of routine microscopic examination of vaginal secretions,quintuple examination of vaginitis and the combined detection of the two in vaginitis.Results:Of the 234 patients with suspected vaginitis,74(31.62％)were diagnosed with vaginitis and 160(68.38％)were non-vaginitis by culture method.The detection rate of vaginal cleanliness Ⅲ and Ⅳ by routine microscopic examination of vaginal secretions combined with quintuple examination of vaginitis was higher than that of single test,and the difference was statistically significant(P<0.05).There was no significant difference in the results of vaginal cleanliness examination between routine microscopic examination of vaginal secretions and quintuple examination of vaginitis(P>0.05).With the culture method as the "gold standard",the sensitivity,specificity,negative predictive value,accuracy,and positive predictive value of the routine microscopic examination of vaginal secretions combined with quintuple examination of vaginitis were higher than those of the single test,and the differences were statistically significant(P<0.05).The consistency between routine microscopic examination of vaginal secretions and culture method results was general(kappa=0.570,P<0.001),and the consistency between the routine microscopic examination of vaginal secretions combined with quintuple examination of vaginitis and culture method results was good(kappa=0.790、0.950,P<0.001).Conclusion:The routine microscopic examination of vaginal secretions combined with quintuple examination of vaginitis can judge vaginal cleanliness more accurately and improve the detection rate of vaginitis.
