摘要

目的:探讨基于聚类分析的视频导向宣教对输尿管结石术后留置双J管患者的影响.方法:选取 2020 年1 月至 2022 年 12 月本院收治的 62 例输尿管结石术后留置双J管患者为研究对象.按照随机数字表法,将其分为干预组(n=31,提供基于聚类分析的视频导向宣教)与常规组(n=31,提供常规健康教育).比较两组患者双J管移位率与非计划性拔管率、引流知识知晓度、心理状态[心理弹性量表(CD-RISC)]、应对方式[医学应对方式问卷(MCMQ)]、自理能力[自理能力评估量表(ESCA)].结果:干预组患者双J管移位率、非计划性拔管率均低于常规组,差异均有统计学意义(P<0.05);干预组患者引流知识知晓度各项评分均高于常规组,差异均有统计学意义(P<0.05);干预组患者CD-RISC评分高于常规组,MCMQ中面对评分高于常规组,回避、屈服评分低于常规组,ESCA评分高于常规组,差异均有统计学意义(P<0.05).结论:基于聚类分析的视频导向宣教干预可降低输尿管结石术后患者置管并发症风险,提升引流知识知晓度,转变心理弹性与应对方式,提高自理能力.

Abstract

Objective:To explore the influence of video-oriented education based on cluster analysis on patients with indwelling double J tube after ureteral calculi surgery.Methods:A total of 62 patients with indwelling double J tube after ureteral calculi surgery who were admitted to our hospital from January 2020 to December 2022 were selected as research objects.They were randomly divided into intervention group(n=31,received video-oriented education based on cluster analysis)and conventional group(n=31,received conventional health education).The incidence rate of double J tube displacement and unplanned extubation rate,drainage knowledge awareness,psychological status[Connor-Davidson resilience scale(CD-RISC)],coping styles[medical coping modes questionnaire(MCMQ)]and self-care ability[evaluation of self-care ability scale(ESCA)]were compared between the two groups.Results:The incidence rate of double J tube displacement and unplanned extubation rate in intervention group were lower than those in conventional group,and the differences were statistically significant(P<0.05).After intervention,the scores of drainage knowledge awareness in intervention group were higher than those in conventional group,and the differences were statistically significant(P<0.05).The CD-RISC score of intervention group was higher than that of conventional group;the MCMQ facing score of intervention group was higher than that of conventional group,the avoidance and yielding score of intervention group were lower than those of conventional group;and ESCA score of intervention group was higher than that of conventional group,and the differences were statistically significant(P<0.05).Conclusion:Video-oriented education intervention based on cluster analysis can reduce the risk of catheter complications in patients after ureteral calculi surgery,improve the awareness of drainage knowledge,change the psychological resilience and coping styles,and enhance the self-care ability.
