分阶段体位干预对椎管内分娩镇痛初产妇的影响

摘要
目的:探讨分阶段体位干预对椎管内分娩镇痛初产妇分娩信心、产程进展和母乳喂养率的影响.方法:选取 2020 年8 月至2023 年8 月本院收治的70 例椎管内分娩镇痛初产妇为研究对象.根据干预方式,将其分为观察组(n=38,采用分阶段体位干预)和常规组(n=32,采用常规分娩干预).比较两组初产妇的分娩信心、分娩疼痛程度、产程进展以及泌乳时间和母乳喂养率.结果:干预后,观察组初产妇的简化中文版分娩自我效能量表(CBSEI-C32)评分高于常规组,视觉模拟评分法(VAS)评分低于常规组,差异均有统计学意义(P＜0.05);观察组初产妇的各产程时间及总产程时间均短于常规组,泌乳时间短于常规组,差异均有统计学意义(P＜0.05),观察组初产妇的母乳喂养率与常规组比较,差异无统计学意义(P＞0.05).结论:通过采用分阶段体位干预手段对分娩镇痛初产妇进行管理,可有效提升产妇的分娩信心,减轻产妇生产过程中的疼痛感,加快生产进程,提高产后的母乳喂养率.
Abstract
Objective:To explore the effects of staged posture intervention on delivery confidence,labor progress and breastfeeding rate in primiparas undergoing intraspinal labor analgesia.Methods:A total of 70 primiparas undergoing intraspinal labor analgesia admitted to our hospital from August 2020 to August 2023 were selected as the research objects.According to intervention methods,they were divided into observation group(n=38,received staged posture intervention)and routine group(n=32,received routine intervention).The delivery confidence,labor pain,labor progress,lactation time and breastfeeding rate were compared between the two groups.Results:After intervention,the score of the short form of Chinese childbirth self-efficacy inventory(CBSEI-C32)in observation group was higher than that of routine group,and the visual analogue scale(VAS)score in observation group was lower than that in routine group,and the differences were statistically significant(P＜0.05).The duration of labor and the total duration of labor in observation group were lower than those in routine group,and the lactation time of observation group was lower than that of routine group,and the differences were statistically significant(P＜0.05),there was no significant difference in breast feeding rate between the two groups(P＞0.05).Conclusion:Staged posture intervention can effectively enhance delivery confidence,relieve labor pain,accelerate the progress of labor and increase postpartum breastfeeding rate in primiparas undergoing intraspinal labor analgesia.
