摘要
目的:探讨结果导向护理对重症心力衰竭患心功能及预后的影响.方法:选取 2021 年 4 月至 2023 年 4月本院心内科重症监护室收治的 86 例重症心力衰竭患者为研究对象.采用随机数字法,将其分为观察组(n=43,实施结果导向护理)和对照组(n=43,实施常规护理).比较两组患者心理状态、自我管理能力、心功能指标以及并发症总发生率.结果:护理后,观察组患者抑郁-焦虑-压力量表(DASS)各项评分均低于对照组,差异均有统计学意义(P＜0.05);观察组患者自我护理能力测定量表(ESCA)各项评分均高于对照组,差异均有统计学意义(P＜0.05);观察组患者左心室舒张末期容积(LVEDV)低于对照组,左心室舒张期内径(LVEDD)短于对照组,差异均有统计学意义(P＜0.05);观察组患者并发症总发生率低于对照组,差异有统计学意义(P＜0.05).结论:结果导向护理可改善重症心力衰竭患者消极情绪,增强自我管理能力,提升心功能水平,减少并发症发生,改善患者预后.
Abstract
Objective:To explore the effect of outcome-oriented nursing on cardiac function and prognosis of patients with severe heart failure.Methods:A total of 86 patients with severe heart failure admitted to the intensive care unit of the Department of Cardiology of our hospital from April 2021 to April 2023 were selected as research objects.They were randomly divided into observation group(n=43,received outcome-oriented nursing measures)and control group(n=43,received routine nursing measures).The mental state,self-management ability,cardiac function and complications were compared between the two groups.Results:After nursing,the scores of depression-anxiety-stress scale(DASS)in observation group were lower than those in control group,and the differences were statistically significant(P＜0.05).After nursing,the scores of exercise of self-care agency scale(ESCA)in observation group were higher than those in control group,and the differences were statistically significant(P＜0.05).After nursing,the left ventricular end-diastolic volume(LVEDV)in the observation group was lower than that in the control group,and the left ventricular end-diastolic diameter(LVEDD)was shorter than that in the control group,and the differences were statistically significant(P＜0.05).The total incidence of complications in observation group was lower than that in control group,and the difference was statistically significant(P＜0.05).Conclusion:Outcome-oriented nursing can alleviate negative emotions in patients with severe heart failure,enhance their self-management ability,improve cardiac function,reduce complications and improve prognosis.
