目的:探讨藻酸钙敷料对超声清创肛周脓肿术后患者创面的影响.方法:选取我科 2020 年 12 月至 2022 年2 月期间收治的 97 例肛周脓肿术后患者作为研究对象,随机分为两组,两组均予以超声清创,其中对照组 48 例在超声清创后予以常规敷料包扎换药,观察组49例在超声清创后予以藻酸钙敷料包扎换药,换药7 d后,对比两组患者创面中细胞因子含量、创面感染控制情况以及创面微循环.结果:换药 7 d后,观察组表皮生长因子、转化生长因子-β、SMAD同源物3(Recombinant SMAD family member 3,SMAD3)、碱性成纤维细胞生长因子含量高于对照组(P<0.05);观察组创面细菌清除率高于对照组,创面愈合时间、感染控制时间短于对照组,排便疼痛程度低于对照组,创面渗出液情况好于对照组(P<0.05);观察组创面血流量、经皮氧分压高于对照组(P<0.05).结论:藻酸钙敷料应用于肛周脓肿术超声清创后患者有助于促进多种增殖细胞因子表达,改善创面微循环和创面感染控制情况,提高创面细菌清除率.
Effect of calcium alginate dressing on the wound after ultrasonic debridement of perianal abscess
Objective:To investigate the effect of calcium alginate dressing on the wound after ultrasonic debridement of perianal abscess.Method:Ninety-seven patients with perianal abscess treated in our department from December 2020 to February 2022 were randomly divided into two groups.Both groups were given ultrasonic debridement.Among them,48 patients in the control group were given conventional dressing and dressing changing after ultrasonic debridement,while 49 patients in the observation group were given calcium alginate dressing and dressing changing after ultrasonic debridement.The contents of cytokines,wound infection control and wound microcirculation in the two groups were compared.Results:7 d after dressing change,the contents of Recombinant growth factor,recombinant growth factor-β,Recombinant SMAD family member 3(SMAD3)and basic fibroblast growth factor in observation group were higher than those in control group(P<0.05).The wound bacterial clearance rate in the observation group was higher than that in the control group,the wound healing time and infection control time were shorter than that in the control group,the degree of defecation pain was lower than that in the control group,and the condition of wound exudation was better than that in the control group(P<0.05).The wound blood flow and percutaneous oxygen partial pressure in the observation group were higher than those in the control group(P<0.05).Conclusion:The application of calcium alginate dressing in patients with perianal abscess after ultrasonic debridement can promote the expression of multiple proliferative cytokines,improve wound microcirculation and wound infection control,and increase the wound bacterial clearance rate.
