目的:研究高血压脑出血患者脑脊液中miR-155(microRNA-155)、miR-126 对预后的影响分析.方法:收集 2020 年 1 月至 2022 年 12 月期间本院收治的 129 例高血压脑出血患者作为研究对象.根据NIHSS(National Institutes of Health Stroke Scale)评分将患者分为重度卒中、中度卒中、轻度卒中三组.收集患者的基线资料.分析对比三组的基线资料.分析入院后第 1 d患者miRNA-155 和miRNA-126 的表达水平,miRNA-155 和miRNA-126 与血肿量、神经元特异性烯醇化酶(neuron-specific enolase,NSE)、神经纤维丝轻链(Neurofilament light chain,NfL)、NIHSS评分的相关性.分析入院后第 14 d患者miRNA-155、miRNA-126与NSE、NfL、APACHE II(Acute Physiology and Chronic Health Evaluation II)评分、mRS(ModifiedRankin Scale)评分、NIHSS评分相关性.绘制ROC(receiver operating characteristic curve)曲线分析miRNA-155、miRNA-126 单独及联合用于检测判断脑出血患者预后的价值.结果:重度卒中组中血肿量、合并糖尿病比例、miRNA-155、miRNA-126、NSE、NfL表达水平与其他两组差异具有统计学意义(P<0.05);miRNA-155 与血肿量、NSE、NfL表达呈反比,miRNA-126 反之(P<0.05).miRNA-155 与NSE、NfL、APACHE II评分呈负相关,miRNA-126 反之(P<0.05).入院 14 d联合miRNA-155 与miRNA-126 对预后预测的敏感度、特异性分别为 95.6%、93.2%.结论:入院 14 d脑脊液中miRNA-155、miRNA-126 在一定程度上反应高血压脑出血患者病情程度,并对患者预后具有一定的临床意义.
Effect of miR-155 and miR-126 expression levels in cerebrospinal fluid on prognosis of hypertensive cerebral hemorrhage patients
Objective:To investigate the effect of miR-155(microRNA-155)and miR-126 in cerebrospinal fluid of patients with hypertensive cerebral hemorrhage on prognosis.Methods:129 patients with hypertensive cerebral hemorrhage admitted to our hospital from January 2020 to December 2022 were collected as research objects.According to the National Institutes of Health Stroke Scale(NIHSS)score,the patients were divided into three groups:severe stroke,moderate stroke and mild stroke.Baseline data of the three groups were analyzed and compared.The expression levels of miRNA-155 and miRNA-126 were analyzed on the first day after admission.Correlation of miRNA-155 and miRNA-126 with hematoma volume,neuron-specific enolase(NSE),Neurofilament light chain(NfL),and NIHSS score.The relationship between miRNA-155,miRNA-126 and NSE,NfL,APACHE II(Acute Physiology and Chronic Health Evaluation II)scores and ModifiedRankin Scale(mRS),NIHSS score correlation were analyzed on the 14th day after admission.The receiver operating characteristic curve(ROC)was drawn to analyze the value of miRNA-155 and miRNA-126 alone and in combination in detecting the prognosis of patients with cerebral hemorrhage.Results:The amount of hematoma,the proportion of diabetes mellitus,the expression levels of miRNA-155,miRNA-126,NSE and NfL in the severe stroke group were significantly different from those in the other two groups(P<0.05).The expression of miRNA-155 was inversely correlated with the amount of hematoma,NSE and NfL,while miRNA-126 was inversely correlated(P<0.05).miRNA-155 was negatively correlated with NSE,NfL and APACHE II scores,while miRNA-126 was negatively correlated with NSE,NFL and Apache II scores(P<0.05).The sensitivity and specificity of miRNA-155 and miRNA-126 for prognosis were 95.6%and 93.2%,respectively,at 14 days after admission.Conclusion:The levels of miRNA-155 and miRNA-126 in CSF 14 days after admission reflect the severity of hypertensive intracerebral hemorrhage to a certain extent,and have certain clinical significance for the prognosis of patients.
