Effects of transdermal estradiol gel combined with Gongankang on preventing intrauterine adhesions after uterine curettage in the second trimester of pregnancy
Objective:To explore the application of transdermal estradiol gel combined with Gongankang(uterine crosslinked sodium hyaluronate gel)in the prevention of intrauterine adhesions after uterine curettage in the second trimester of pregnancy.Methods:Seventy-one patients who received artificial abortion or induced labor in the hospital from January 2021 to November 2022 were selected and randomly divided into the combined group(n=35)and the control group(n=36)according to the odd-even number method.The patients in the control group received 0.05 g·kg-1 of Gongankang and 3 days of conventional anti-infection treatment,and the patients in the combined group were treated with 0.05 g·kg-1 of transdermal estradiol gel once a day based on the control group.After 2 months of treatment,hysteroscopy was used to observe the incidence rate and severity of intrauterine adhesions.Hormones including luteinizing hormone(LH),follicle-stimulating hormone(FSH),and estradiol(E2)levels were measured by chemiluminescence assay.The levels of interleukin-10(IL-10),interleukin-6(IL-6)and interleukin-4(IL-4)were measured by enzyme method.The endometrial thickness was measured by vaginal B-ultrasound,and the menstruation recovery time,menstruation volume,and occurrence of adverse reactions after treatment were recorded and compared between the two groups.Results:After treatment,the incidence rate of intrauterine adhesions and menstruation recovery time in the combined group were significantly lower or shorter than those in the control group(P<0.05).Compared with before treatment,the levels of LH,FS,E2,IL-4,and IL-10 and endometrial thickness in both groups were significantly increased(P<0.05),and the above indicators were more significant in the combined group(P<0.05).The level of IL-6 was significantly decreased in both groups(P<0.05),and the change was more obvious in the combined group(P<0.05).After treatment,the adverse reactions were similar in the two groups(P>0.05).Conclusion:The application of transdermal estradiol gel after uterine curettage in the second trimester of pregnancy can reduce the risk of intrauterine adhesions,relieve the inflammatory response,and improve the uterine environment.
