Application of different imaging methods in the evaluation of cervical spine function and clinical efficacy in patients with cervical spondylotic myelopathy
Objective:To explore the application of different imaging methods in the evaluation of cervical spine function and clinical efficacy in patients with cervical spondylotic myelopathy.Methods:One hundred and twenty patients with cervical spondylotic myelopathy in the hospital from January 2020 to January 2023 were selected as observation group,and one hundred and three healthy people who received physical examination during the same period were enrolled as control group.Both groups were examined by computerized tomography(CT),magnetic resonance diffusion tensor imaging(DTI)and Japanese Orthopedic Association(JOA)score.CT quantitative parameters(bony spinal canal volume,fibrous spinal canal volume and spinal canal volume occupancy rate)and DTI parameters[apparent dispersion coefficient(ADC),fractional anisotropy(FA)]between patients with cervical spondylotic myelopathy and healthy people and CT quantitative parameters and DTI parameters among patients with cervical spondylotic myelopathy before and after surgery were compared.According to the clinical efficacy,120 patients with cervical spondylotic myelopathy were divided into effective group(n=97)and ineffective group(n=23),and CT quantitative parameters and DTI parameters were compared between both groups,and the relationship between imaging parameters and JOA score was analyzed.Results:The bone spinal canal volume,fibrous spinal canal volume,spinal canal volume occupancy rate and FA in observation group were lower than those in control group while the ADC was higher than that in control group(P<0.05).After cervical discectomy and fusion or cervical corpectomy and bone graft fusion,the bone spinal canal volume,fibrous spinal canal volume,spinal canal volume occupancy rate and FA of patients with cervical spondylotic myelopathy were higher while the ADC was lower compared to before surgery(P<0.05).The postoperative fibrous spinal canal volume,spinal canal volume occupancy rate and FA in effective group were higher than those in ineffective group while the ADC value was lower compared to ineffective group(P<0.05),but the bone spinal canal volume in the two groups was similar(P>0.05).The fibrous spinal canal volume,spinal canal volume occupancy rate and FA of patients with cervical spondylotic myelopathy after surgery were positively correlated with JOA score(r=0.422,0.403,0.539,P<0.05),and ADC was negatively correlated with JOA score(r=-0.598,P<0.05).Conclusion:Both CT and DTI can effectively reflect the cervical spine function and clinical efficacy of patients with cervical spondylotic myelopathy.
