目的:研究地塞米松棕榈酸酯联合利多卡因治疗肩袖损伤患者的临床疗效.方法:回顾性分析 2019 年 8 月至 2022年 10 月期间本院康复科收治的 78 例肩袖损伤患者的临床资料,按照治疗方法不同将患者分为对照组(肩关节镜下手术治疗,37 例)和观察组(肩关节镜下手术治疗基础上采用地塞米松棕榈酸酯联合利多卡因治疗,41例).治疗 1 m后以美国加州大学肩关节评分量表(the university of California at Los Angeles shoulder rating scale,UCLA)评估肩关节功能,采用酶联免疫吸附试验法检测白细胞介素 6(Interleukin-6,IL-6)、C 反应蛋白(C reactive protein,CRP)水平,同时观察药物安全性.结果:治疗后,两组UCLA各维度评分均升高,且观察组UCLA各维度评分均显著高于对照组(P＜0.05).治疗后,两组炎性因子IL-6、CRP水平均降低,且观察组的IL-6、CRP水平均显著低于对照组(P＜0.05).治疗后,两组不良反应总发生率比较无显著差异(P＞0.05).结论:地塞米松棕榈酸酯联合利多卡因治疗肩袖损伤的临床疗效显著,可减轻机体的炎症反应状态,改善肩关节功能.
Clinical efficacy of dexamethasone palmitate combined with lidocaine in the treatment of rotator cuff injury
Abstact Objective:To study the clinical efficacy of dexamethasone palmitate combined with lidocaine in the treatment of rotator cuff injury.Methods:The clinical data of 78 patients with rotator cuff injury admitted to the rehabilitation department of our hospital from August 2019 to October 2022 were retrospectively analyzed.According to different treatment methods,the patients were divided into control group(shoulder arthroscopic surgery,37 cases)and observation group(dexamethasone palmitate combined with lidocaine on the basis of shoulder arthroscopic surgery,41 cases).After 1 month of treatment,shoulder function was assessed by the university of California at Los Angeles shoulder rating scale(UCLA).The levels of Interleukin-6(IL-6)and C reactive protein(CRP)were detected by enzyme-linked immunosorbent assay(ELISA),and the safety of the drug was also observed.Results:After treatment,the scores of all dimensions of UCLA in both groups were increased,and the scores of all dimensions of UCLA in observation group were significantly higher than those in control group(P<0.05).After treatment,the levels of inflammatory factor IL-6 and CRP were decreased in both groups,and the levels of IL-6 and CRP in observation group were significantly lower than those in control group(P<0.05).After treatment,there was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:Dexamethasone palmitate combined with lidocaine is effective in the treatment of rotator cuff injury,which can reduce the inflammatory state of the body and improve the shoulder joint function.
