Evaluation of QRS changes in electrocardiogram in patients with coronary heart failure
Objective:To investigate the application of ECG QRS changes in the assessment of patients with coronary heart failure.Methods:106 patients with CHD and heart failure admitted to our hospital from January 2020 to January 2023 were selected as the study objects, and were divided into 61 patients with QRS duration≥110ms group and 45 patients with QRS duration<110ms group according to different ECG QRS duration. The cardiac function grades of the two groups were analyzed and compared. The indexes of cardiac function, such as Left ventricular ejection fraction(LVEF),Left ventricular end diastolic dimension(LVDD),Left Ventricular Fraction Shortening(LVFS)and the association of QRS wave groups with cardiac function grading and cardiac function. Results:The grade Ⅱ component ratio of QRS wave duration≥110 ms group was significantly lower than that of QRS wave duration<110 ms group, the grade Ⅳ component ratio of QRS wave duration<110 ms group was significantly higher than that of QRS wave duration<110 ms group(P<0.05),there was no statistical significance in the comparison of grade Ⅲ cardiac function between the two groups(P>0.05).LVEF and LVFS in QRS group≥110 ms were significantly higher than those in QRS group<110 ms,and LVDD was significantly lower than those in QRS group<110 ms(P<0.05).Pearson correlation analysis showed that QRS group duration of patients with heart failure was negatively correlated with cardiac function grade(r=0.713,P<0.001)and LVDD(r=0.697,P<0.001),and was negatively correlated with LVEF(r=-0.729,P<0.001)and LVFS(r=-0.708,P<0.001).P<0.001)positive correlation. Conclusion:The longer the duration of QRS wave group, the higher the cardiac function grade and LVEF,LVFS,and the lower LVDD in patients with heart failure. The prolonged duration of QRS wave is correlated with cardiac function grade and LVDD in patients with heart failure.
