STIR联合FLAIR对视神经炎临床诊断的作用分析

目的:探讨短时间反转恢复序列(Short Time of Inversion Recovery,STIR)联合液体衰减反转恢复序列(Fluid-attenuated inversion recovery,FLAIR)对视神经炎(Optic neuritis,ON)临床诊断作用.方法:回顾性分析2018年 1 月至 2022 年 12 月间我院收治的 82 例确诊为ON且均为单眼受累的患者的临床资料.所有患者均接受磁共振扫描,包括冠状面STIR、STIR-FLAIR以及T1WI序列.记录STIR、STIR-FLAIR视神经炎检出率、病变部位检出情况以及标准信号强度(Standard signal strength,Sn)、病变信号强度/同侧眼眶内脂肪信号强度(Signal intensity of ON lesions/ipsilateral orbital fat signal,SON/SIfat)、病变信号强度/对侧视神经信号强度(Signal intensity of ON lesions/contralateral optic nerve signal intensity,SON/SCON)指标.结果:STIR-FLAIR序列检出率高于STIR序列(P<0.05);STIR序列病变部位分为眶内段 30 个、管内段 19 个、眶内和管内段 22 个、眶内-管内-颅内 6 个;STIR-FLAIR序列病变部位分为眶内段 32 个、管内段 20 个、眶内和管内段 23 个、眶内-管内-颅内 7 个,两组序列病变部位检出数据差值无意义;STIR-FLAIR序列的Sn、SON/SIfat、SON/SCON均高于STIR序列(P<0.05).结论:相较STIR序列而言,STIR-FLAIR序列对脂肪信号、水序号的抑制优势明显,可以提高ON的临床诊断可靠性,值得推广.
Effect of STIR combined with FLAIR on the clinical diagnosis of optic neuritis
Objective:Explore the clinical diagnostic effect of Short Time of inversion recovery(STIR)combined with fluid-attenuated inversion recovery(FLAIR)on optic neuritis(ON).Methods:Clinical data of 82 patients diagnosed with ON in our hospital from January 2018 to December 2022,all of whom were involved in one eye,were retrospectively analyzed.All patients underwent MRI scans including coronal STIR,STIR-FLAIR,and T1WI sequences.STIR,STIR-FLAIR optic neuritis detection rate,lesion site detection and Standard signal strength(Sn)were recorded,signal intensity of ON lesions/ipsilateral orbital fat signal(SON/SIfat),signal intensity of ON lesions/contralateral optic nerve signal intensity(SON/SCON).Results:The detection rate of STIR FLAIR sequence was higher than that of STIR sequence(P<0.05).STIR sequence lesions were divided into 30 intraorbital segments,19 intracranial segments,22 intraorbital and intracranial segments,and 6 intraorbital,intracranial and intracranial segments.STIR-FLAIR lesions were divided into 32 intraorbital segments,20 intracranial segments,23 intraorbital and intracranial segments,and 7 intraorbital-intracranial lesions.The difference between the two groups was significant.Sn,SON/SIfat and SON/SCON in STIR-FLAIR sequences were higher than those in STIR sequences(P<0.05).Conclusion:Compared with STIR sequence,STIR-FLAIR sequence has obvious advantages in inhibiting fat signal and water sequence number,which can improve the reliability of clinical diagnosis of ON and is worth promoting.
