经鼻高流量湿化氧疗对AECOPD患者临床疗效、血气分析及炎性因子指标的影响

目的:分析经鼻高流量湿化氧疗(High-flow nasel cannula oxygen therapy,HFNC)对慢性阻塞性肺疾病急性加重期(Acute exacerbation of chronic obstructive pulmonary disease,AECOPD)患者临床疗效、血气分析及炎性因子指标的影响.方法:回顾性分析 2021 年 5 月至 2023 年 10 月我院收治的AECOPD患者 83 例为研究对象,根据治疗方法的不同分为对照组(常规氧疗,n=40)和观察组(采用 HFNC 治疗,n=43).对比两组临床疗效,并于治疗 1 w后取动脉血进行血气分析,包括酸碱度(Potential of hydrogen potential of hydrogen,pH)、动脉血氧分压(Arterial partial pressure of oxygen,PaO2)、动脉二氧化碳分压(Partial pressure of carbon dioxide,PaCO2),采用循环增强荧光免疫发光法及酶联免疫吸附法测定白介素-6(Interleukin,IL-6)、白介素-10(Interleukin,IL-10)、肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)水平.结果:观察组有效率为 81.40%,对照组有效率为 65.00%,观察组有效率高于对照组(P<0.05);两组治疗 1 w后PaO2明显升高、PaCO2 明显降低,且观察组治疗后PaO2高于对照组,PaCO2 低于对照组(P<0.05);治疗 24 h后,两组血清IL-6、IL-10、TNF-α水平均降低,且观察组血清IL-6、IL-10、TNF-α水平均低于对照组(P<0.05).结论:HNFC可提高AECOPD患者的治疗疗效,有效改善血气分析以及炎性因子水平,值得临床推广应用.
Effects of nasal high-flow humidified oxygen therapy on clinical efficacy,blood gas analysis and inflammatory factors in patients with AECOPD
Objective:Analysis of High-flow nasel cannula oxygen therapy,HFNC)on Acute exacerbation of chronic obstructive pulmonary disease period(Acute exacerbation of chronic obstructive pulmonary diseases,AECOPD)in patients with clinical curative effect,the influence of blood gas analysis and inflammatory factor index.Methods:A retrospective analysis was performed on 83 patients with AECOPD treated in our hospital from May 2021 to October 2023,who were divided into control group(conventional oxygen therapy,n=40)and observation group(HFNC treatment,n=43)according to different treatment methods.The clinical efficacy of the two groups was compared,and arterial blood was taken after 1 w of treatment for blood gas analysis,including Potential of hydrogen potential of hydrogen,pH,Arterial partial pressure of oxygen(PaO2),arterial Partial pressure of carbon dioxide(PaCO2),The levels of interleukin-6(IL-6),interleukin-10(IL-10)and Tumor necrosis factor-α(TNF-α)were measured by cyclic enhanced fluorescence immunoluminescence and enzyme-linked immunosorbent assay.Results:The effective rate was 81.40%in the observation group and 65.00%in the control group,which was higher than that in the control group(P<0.05).After 1 w of treatment,PaO2 was significantly increased and PaCO2 was significantly decreased in both groups,and PaO2 in observation group was higher than that in control group and PaCO2 was lower than that in control group(P<0.05).After 24 h of treatment,the levels of serum IL-6,IL-10 and TNF-α in both groups were decreased,and the levels of serum IL-6,IL-10 and TNF-α in observation group were lower than those in control group(P<0.05).Conclusion:HNFC can improve the therapeutic effect of AECOPD patients,improve blood gas analysis and inflammatory factors effectively,and is worthy of clinical application.
