Relationship between salivary CA-Ⅵ,MMP-2,CCL28 and caries severity in children and their value in predicting caries recurrence
Objective:To investigate the effects of salivary carbonic anhydrase Ⅵ(CA-Ⅵ),matrix metalloproteinase-2(MMP-2),and CC chemokine ligand 28(CCL28)levels in determining the caries condition and predicting the recurrence of caries in children.Methods:86 patients with caries who were admitted to our hospital from April 2019 to April 2022 and completed 1-year follow-up were selected as the study objects.According to the caries loss index(DMFT),the patients were divided into low caries group(39 cases,DMFT= 1-4)and high caries group(47 cases,DMFT≥5).The patients were divided into recurrence(24 cases)and non-recurrence(62 cases)according to the recurrence rate 1 year after treatment.The levels of CA-Ⅵ,MMP-2,CCL28 in saliva and their correlation with DMFT were compared between the low caries group and the high caries group.The levels of CA-Ⅵ,MMP-2 and CCL28 in saliva of patients with and without relapse were compared.The risk of recurrence of caries at CA-Ⅵ,MMP-2 and CCL28 levels was analyzed.The values of CA-Ⅵ,MMP-2 and CCL28 levels in predicting caries recurrence were analyzed.Results:Salivary CA-Ⅵ level in the high caries group was lower than that in the low caries group at the time of admission,which was negatively correlated with DMFT;salivary MMP-2 and CCL28 levels in the high caries group were higher than those in the low caries group,which were positively correlated with DMFT(P<0.05).Salivary CA-Ⅵ levels in recurrence group were significantly lower than those in non-recurrence group at the time of admission and at 1 month after treatment;and the levels of MMP-2 and CCL28 in recurrence group were significantly higher than those in non-recurrence group(P<0.05).The AUC for the combined prediction of caries relapse of CA-Ⅵ,MMP-2,and CCL28 levels was 0.808,and the optimal sensitivity and specificity were 91.67%and 77.42%,respectively.Conclusion:The changes of CA-Ⅵ,MMP-2 and CCL28 levels in saliva can provide reference for the diagnosis of caries in children,and have important significance for prognosis assessment.
