Application of MSCT in preoperative diagnosis and differential diagnosis of lung cancer and imaging characterisation
Objective:To analyze the application and imaging features of Multi-Slice CT(MSCT)in the preoperative diagnosis and differential diagnosis of lung cancer.Methods:One hundred and twenty-one lung cancer patients admitted to our hospital from March 2020 to July 2023 were selected to compare the diagnostic results of MSCT and pathological examination with pathological examination as the"gold standard",and the consistency between MSCT and pathological examination was analyzed by kappa test.Judgement was made with reference to the TNM staging criteria,and the T-stages of MSCT examination and pathological examination were compared.To analyze the imaging characteristics of MSCT in lung cancer(peripheral lung cancer,central lung cancer).Results:Taking pathological examination as the"gold standard",47 cases of peripheral lung cancer and 74 cases of central lung cancer were detected by MSCT;7 cases were missed/misdiagnosed,with a diagnostic accuracy of 94.21%,and the concordance with the kappa test of pathological examination was 0.68(P<0.05).The accuracy of MSCT examination for preoperative diagnosis of lung cancer stages T1-T2,T3 and T4 was 90.38%,94.54%and 89.63%,respectively.MSCT images of 43 patients with peripheral lung cancer showed the phenomenon of lobular sign in 30 cases,pleural depression and vascular cluster sign in 19 cases,burr sign in 27 cases,and the presence of vacuolar sign and bronchial insufflation sign in 13 cases.78 MSCT image of central lung cancer shows irregular or ring-shaped hilar mass;the mass shows the phenomenon of lobar sign;the hilar shadow increases and thickens with the change of marginal umbilical fossa;the bronchial wall is thickened and the lumen is narrowed;the bronchial tubes show the truncation sign;and the enhancement scan shows mild to moderate enhancement,which is inhomogeneous.Conclusion:MSCT examination has high diagnostic efficacy in preoperative diagnosis and differential diagnosis of lung cancer,and MSCT examination can completely show the location,size,morphology and other characteristics of the patient's lesion,which can provide the basis for preoperative TNM staging.
