Efficiency of high-frequency ultrasound combined with mammography in the differential diagnosis of plasmocytic mastitis and granulomatous mastitis
Objective:To investigate the differential diagnosis of plasma cell mastitis and granulomatous mastitis by high-frequency ultrasound combined with molybdenum target X-ray,in order to provide reference for early clinical diagnosis and treatment.Methods:100 cases of mastitis treated in Zhengzhou Hospital of Traditional Chinese Medicine from May 2022 to May 2023 were examined by high-frequency ultrasound and molybdenum target X-ray.Using the final pathological examination results as the"gold standard",the ultrasonographic features of plasmocytic mastitis and granulomatous mastitis were compared. The differential diagnosis results and diagnostic efficiency of high frequency ultrasound, molybdenum target X-ray and combined examination were compared. Results:Pathological examination showed that 68 cases of plasma cell mastitis and 32 cases of granulomatous mastitis were among 100 cases of mastitis. The results of high-frequency ultrasonography showed that there were 56 cases of plasma cell mastitis and 44 cases of granulomatous mastitis.Molybdenum target X-ray examination results showed that 54 cases of plasma cell mastitis and 46 cases of granulomatous mastitis. The combined results of high-frequency ultrasound and molybdenum target X-ray showed that 67 cases of plasma cell mastitis and 33 cases of granulomatous mastitis. There were statistically significant differences between plasmocytic mastitis and granulomatous mastitis in the ultrasonographic features of mass area, surface fistula, contralateral catheter dilation, and subaxillary lymph nodes(P<0.05).The sensitivity(94.12%)and accuracy(93.00%)of high-frequency ultrasound combined with molybdenum target X-ray in the differential diagnosis of plasmocytic mastitis and granulomatous mastitis were 64/68,93.00%(93/100).The sensitivity and accuracy were 79.41%(54/68)and 84.00%(84/100)respectively,and the sensitivity and accuracy of molybdenum target X-ray examination were 75.00%(51/68)and 80.00%(80/100)respectively.The missed diagnosis rate of combined examination was 5.88%(4/68).It was lower than that of high frequency ultrasound alone(20.59%(14/68)and molybdenum target X-ray alone(25.00%(17/68)(P<0.05).Conclusion:High frequency ultrasound combined with molybdenum target X-ray examination has high diagnostic efficacy in the differential diagnosis of plasmocytic mastitis and granulomatous mastitis, which can be used for the early diagnosis of mastitis, providing reference for the targeted treatment plan, and contributing to the improvement of the prognosis of mastitis.
